Endothelin-1（EDN1）は正常表皮の基底細胞に限局的に強く発現され，基底細胞癌でもその発現が認められることが報告されている。しかし基底細胞癌を二次的に発生しやすい脂腺母斑での EDN1 発現はこれまで検討されていない。我々は脂腺母斑 10 例，基底細胞癌を発生した脂腺母斑 4 例，脂腺母斑周囲の正常皮膚 9 例，およびランダム生検にて採取した正常皮膚 10 例のパラフィン包埋標本を用いて，EDN1 の発現を免疫組織学的に検討した。EDN1 は正常表皮や上部毛囊脂腺系の基底細胞，エクリン汗腺の導管部と分泌部に強い発現を認めた。下部毛囊では，内毛根鞘細胞，毛幹，毛母，毛乳頭は EDN1 陰性であった反面，外毛根鞘細胞は全体的に EDN1 陽性を示した。脂腺母斑表皮でも基底細胞は EDN1 強陽性であった。正常皮膚に比べ，脂腺母斑では肥厚した有棘細胞層内とりわけ毛囊分化を示しつつある部位には EDN1 陽性有棘細胞が有意に増数集簇していた。基底細胞癌が発生した脂腺母斑の基底細胞癌は EDN1 陽性であった。また基底細胞癌から離れた脂腺母斑部に比べ，基底細胞癌近傍の脂腺母斑部では ...

我々は，異なる臨床像を呈した 3 例の Eccrine Angiomatous Hamartoma（EAH）を経験した。症例 1： 78 歳，男性。幼少期から右肘窩部に暗赤色腫瘤を自覚し，緩徐に増大傾向であった。症例 2：41 歳，男性。10 年前から左頰部に淡青灰色局面を自覚していた。局面の大きさは 10 年でほぼ変わりなかった。症例 3：41 歳，女性。8 年前から上腹部に皮下腫瘤を自覚し，2 カ月前から疼痛を伴うようになった。症例 1 と 3 は全切除生検，症例 2 は部分生検を行った。3 症例とも病理組織学的に腫瘍の境界は不明瞭で，真皮にエクリン汗腺や小血管の増生を認め，EAH と診断した。 EAH は比較的まれな腫瘍であり，多彩な臨床像をとる。自験例は初診時に鑑別疾患として EAH は挙がらず，病理組織学的検査で診断がついた。疾患に特徴的な画像所見もこれまで報告されておらず，診断には発症時期や局所多汗などの特徴的な所見を問診することと，皮膚生検が有用と考える。

亜鉛は人体における必須微量元素の中で 2 番目に多く，多数の蛋白構造や機能維持に関与し，酵素の活性・調整因子である。亜鉛は主に小腸で吸収され，生体内の様々な臓器に分布するが，皮膚は生体内で 3 番目に亜鉛を多く含有する臓器であり，皮膚の恒常性維持に亜鉛は必須である。亜鉛が欠乏すると様々な症状を引き起こし，その中でも皮膚において多岐にわたる症状を引き起こす。生体には亜鉛の恒常性維持のために調節機構が備わっているが，その主軸となるものが亜鉛トランスポーターである。Transient neonatal zinc deficiency（TNZD）は母親の SLC30A2 遺伝子変異によって，乳腺細胞の亜鉛トランスポーター ZnT（Zn transporter）2 に機能障害が生じ，乳汁中への亜鉛分泌量が低下し，低亜鉛母乳になる。乳児は通常，母乳から必要量の亜鉛を得ているため，母乳中の亜鉛量が減少した低亜鉛母乳を授乳していると，直ちに亜鉛欠乏に陥る。症例は完全母乳で育児中の生後 4 カ月の男児で生後 2 カ月より後頭部に紅斑が出現した。徐々に口周囲や肛門周囲に鱗屑を伴う紅斑が拡大したため，当科を受診した。 ...
