Background Demand for colonoscopy and EGD procedures is increasing. Impediments to performing these examinations persist. Patients perceive these procedures as unpleasant and painful. The use of suboptimal sedatives results in inefficiency in endoscopy practices. Improving sedation methods utilizing precise control of preferred sedatives may increase patient satisfaction and practice efficiency. Objective Our purpose was to demonstrate the feasibility of computer-assisted personalized sedation (CAPS) ...

Background and objective: We conducted an open, prospective, randomized study to compare the efficacy, safety and recovery characteristics of remifentanil or propofol during monitored anaesthesia care in patients undergoing colonoscopy. Methods: Forty patients were randomly assigned to receive either propofol (1 mg kg−1 followed by 10 mg kg−1h−1, n = 20) or remifentanil (0.5 μg kg−1 followed by 0.2 μg kg−1 min−1, n = 20). The infusion rate was subsequently adapted to clinical needs. Results: In the propofol ...

Mutants of Anabaena sp. strain PCC 7120 that are incapable of sustained growth with air as the sole source of nitrogen were generated by using Tn5-derived transposons. Nitrogenase was expressed only in mutants that showed obvious morphological signs of heterocyst differentiation. Even under rigorously anaerobic conditions, nitrogenase was not synthesized in filaments that were unable to develop heterocysts. These results suggest that competence to synthesize nitrogenase requires a process that leads ...

Internal cardioverter–defibrillator implantation can be performed under local or general anaesthesia. Whether the technique of general anaesthesia influences the defibrillation threshold remains a matter of debate. We therefore compared, in a prospective, randomised clinical study, the effect of intravenous anaesthesia using propofol with inhalational anaesthesia using isoflurane on the defibrillation threshold in 68 patients scheduled for transvenous single‐lead internal cardioverter–defibrillator ...

Both N 6 ,O 2 ′-dibutyryl adenosine-3′,5′-cyclic monophosphate (DBcAMP) and prostaglandin E 1 (PGE 1 ) stimulate maturation and differentiation of neuroblastoma cells in tissue culture. This effect is potentiated by several phosphodiesterase inhibitors. DBcAMP and adenosine-3′,5′-cyclic monophosphate (cAMP) have also been reported to mimic the effects of nerve growth factor (NGF) on dorsal root ganglia but not on sympathetic ganglia in stimulating neurite outgrowth. We found that prostaglandins E 1 and E ...

In the mammalian cochlea acetylcholine has been considered a major neurotransmitter of the lateral and medial efferent fibers. The aims of the present study were to investigate the expression of ChAT in the human cochlea and to develop a new method for immunohistochemical investigations in the human cochlea both at the light and electronmicroscopic level. We thus examined the ChAT-like immunoreactivity in the human inner ear using light and electron microscopy with a pre-embedding technique. Our ...

The combination of propofol-remifentanil for procedural deep sedation in spontaneously breathing patients is characterized by the frequent incidence of side effects, especially respiratory depression. These side effects may be due to either the drug combination or the drug delivery technique. Target-controlled infusion (TCI) might optimize drug delivery. In this prospective, randomized, double-blind study in patients undergoing elective colonoscopy, we thus tried to answer two questions: first, if ...

Enzyme replacement therapy (ERT) with laronidase (recombinant human α-L-iduronidase, Aldurazyme®) is indicated for non-neurological signs and symptoms of mucopolysaccharidosis type I (MPS I). The approved laronidase dose regimen is weekly infusions of 0.58mg/kg, however, patients and caregivers may have difficulty complying with the weekly regimen. Olivocochlear efferent neurons originate in the superior olivary complex of the brainstem and terminate within sensory cell regions of the organ of Corti. Components of this complex include the lateral olivocochlear bundle whose unmyelinated axons synapse with radial afferent dendrites below inner hair cells and the medial olivocochlear bundle, from which myelinated axons form a direct synaptic contact with outer hair cells. γ-Aminobutyric acid (GABA), a major neurotransmitter of the central nervous ...

MOYAMOYA DISEASE (MMD) is a progressive, chronic cerebrovascular occlusive disease occurring predominantly in the Asian population. As described by Suzuki and Takaku1 in 1969, it affects the internal carotid arteries and anterior and middle cerebral arteries, resulting in compensatory collateral networks at the base of the brain. These networks can be observed in cerebral angiography as a "puff of smoke" (moyamoya means "cloud" in Japanese). The clinical appearance is marked by various cerebrovascular ...

New approaches to evidence synthesis, which use human effort and machine automation in mutually reinforcing ways, can enhance the feasibility and sustainability of living systematic reviews. Human effort is a scarce and valuable resource, required when automation is impossible or undesirable, and includes contributions from online communities ("crowds") as well as more conventional contributions from review authors and information specialists. Automation can assist with some systematic review tasks, ...

A living systematic review (LSR) should keep the review current as new research evidence emerges. Any meta-analyses included in the review will also need updating as new material is identified. If the aim of the review is solely to present the best current evidence standard meta-analysis may be sufficient, provided reviewers are aware that results may change at later updates. If the review is used in a decision-making context, more caution may be needed. When using standard meta-analysis methods, ...

While it is important for the evidence supporting practice guidelines to be current, that is often not the case. The advent of living systematic reviews has made the concept of "living guidelines" realistic, with the promise to provide timely, up-to-date and high-quality guidance to target users. We define living guidelines as an optimization of the guideline development process to allow updating individual recommendations as soon as new relevant evidence becomes available. A major implication of ...

Systematic reviews are difficult to keep up to date, but failure to do so leads to a decay in review currency, accuracy, and utility. We are developing a novel approach to systematic review updating termed “Living systematic review” (LSR): systematic reviews that are continually updated, incorporating relevant new evidence as it becomes available. LSRs may be particularly important in fields where research evidence is emerging rapidly, current evidence is uncertain, and new research may change policy ...
