W Steinbrunn, Keith Cohn, Arthur Selzer,

Five patients are described with large, congenital secundum atrial septal defects combined with acquired rheumatic mitral stenosis. In one additional patient mitral stenosis was associated with a small patency of the foramen ovale, perhaps secondary to atrial wall stretching. Clinically, these cases of Lutembacher's syndrome may resemble pure mitral stenosis in some instances and pure atrial septal defect in others. The dangers of incomplete diagnosis and surgery are emphasized: Increased pressure ...

Abstract The purpose of this study was to clarify the relationship between hemodynamic abnormalities found in patients with angina pectoris and the extent of coronary artery disease. Studies were made of 122 patients with anginal-syndrome. Coronary arteriorgrams, hemodynamic studies at rest and during exercise were used and, in the majority (87 patients), treadmill stress tests were performed. On the basis of coronary angiograms, the patients were classified into 3 groups: single-vessel disease (29 ...

Papillary muscle infarction was produced in 16 mongrel dogs by placing sutures around the base of one of the papillary muscles. In addition, patchy infarction of the adjacent left ventricular wall was produced by placing an Ameroid constrictor around the appropriate coronary artery. Mitral insufficiency developed in 14 of these animals; it was severe in four and mild to moderate in ten. Mitral insufficiency was not produced by isolated infarction of a papillary muscle or by isolated infarction of ...

ALBERT A. DUBIN, Harold W. March, Keith Cohn, Arthur Selzer,

Forty-two patients with pure mitral stenosis underwent a prospective, longitudinal study with two or more cardiac catheterizations 1 to 10 years apart and without intervening surgical treatment. The majority of patients had not previously had mitral valve surgery; in 16, the period of observation followed valvotomy. Twenty-seven patients showed evidence of progressive obstruction of the mitral valve; 15 showed no change in the calculated mitral valve area. In the group of patients with progressive ...

Eight patients with chronic congestive heart failure (four with cardiomyopathy and four with ischemic heart disease) underwent hemodynamic studies during acute administration of digoxin, given intravenously in two 0.5 mg doses 2 hours apart. Observations were made before administration of digitalis (control period) and serially thereafter for 4 hours after the first dose. Resting mean cardiac index and pulmonary arterial wedge pressure were as follows: 2.0 liters/min per m2 and 23 mm Hg (control period); ...

To evaluate the fate of the boronary arteries after aortocoronary bypass, 40 patients underwent serial selective coronary angiographic studies 1 year apart, and the frequency of progression of coronary artery disease was estimated. Thirty-two had saphenous vein bypass surgery after the first procedure, six had Vineberg operations, and two had no interim operation. In each patient, the right, left, anterior descending and circumflex coronary arteries (including their branches) were separately evaluated. ...

High resolution 1 H NMR spectra of perfused rat hearts have been obtained under normoxic, ischemic and hypoxic conditions. Several myocardial metabolites including taurine, carnitine, lactate and tissue glycerides are detected in the 1 H NMR spectra. Changes in oxygen availability induce perturbations in the levels of some metabolites, in particular, lactate. Experiments with fasted rats and with substrate‐free perfusion suggest that the glycerides detected in 1 H spectra are metabolically mobilizable ...

Abstract Lead concentrations (μg/g wet weight) in human bone (tibia) were measured noninvasively in vivo employing an X‐ray fluorescence technique. Forty‐five workers who had been subjected to chronic industrial exposure were found to have a mean bone lead content of 52.9 μg/g wet weight (0 to 198 μg/g). In addition to bone lead content, blood lead, body burden of lead as assessed by urinary lead excretion after EDTA chelation, zinc protoporphyrin, and unstimulated urinary lead excretion were evaluated. ...

The objective of this study was to determine factors associated with prescription of highly active antiretroviral therapy (HAART). The authors observed 9530 patients eligible for antiretroviral therapy (ART) in more than 100 hospitals and clinics in 10 US cities. Multiple logistic regression analysis was used to assess factors associated with HAART prescription, stratifying patients by no history versus history of ART to assess the association between prescription and CD4, viral load, and outpatient ...

We sought to evaluate the effect of the aldosterone synthase promoter polymorphism on heart failure outcomes for subjects in the African American Heart Failure Trial (A-HeFT). Genetic heterogeneity modulates clinical outcomes in subjects with heart failure (HF); however, little data exist in African American populations. A common polymorphism exists in the promoter region of the aldosterone synthase gene (CYP11B2) at position −344 (T/C). The −344C allele, associated with higher aldosterone synthase activity, ...
