Sleep attacks, while being characteristic of narcolepsy, have been reported in several metabolic and central nervous system disorders--most of which are refractory to treatment. We have recently treated a 47-year-old man with disabling sleep attacks who was found to have atlantoaxial dislocation. Despite narrowing of the sagittal cervical canal diameter to 12 mm (behind the dens), symptoms of spinal cord compression were mild and intermittent, while signs were absent. Posterior fusion of the C-1 ...

The histopathologic data for lung carcinoma were reviewed in 4928 patients between 1964 and 1985. The analysis of this data indicates that lung cancer is increasing in our region. Squamous carcinoma remains the most frequent type in men, followed by adenocarcinoma. Small cell carcinoma is the second most frequent type in women, after adenocarcinoma. Small cell carcinoma has increased over the past years and may become the most common type of lung cancer in women, a development that might result ...

Purpose: This study established the published prediction equations for the energy expenditure of walking and running compared with the measured values. To make this comparison we first determined whether differences exist in energy expenditure for 1600 m of walking versus running, and whether energy expenditure differences occur due to being on the track or treadmill. Methods: Energy was measured via indirect calorimetry in 24 subjects while walking (1.41 m·s−1) and running (2.82 m·s−1) 1600 m on ...

The purpose of this study was (1) to determine if overweight/obese individuals (age 26-50 y) would self-select moderate exercise intensity when asked to do so and (2) to determine how this self-selected workload compared to exercising at a workload (60% peak aerobic capacity) that is known to provide cardioprotective health benefits. Oxygen consumption (VO(2)) and energy expenditure were measured in 33 men/women (BMI ≥ 27 kg/m(2)) who completed two 30 min walking bouts: (1) self-selected walking pace on ...

Prosthetic treatment of bizarre congenital anomalies of the extremities in over 170 children was simplified by a classification based on the embryological somitic origin of the limb, well known to anatomists but heretofore ignored by clinicians. X-ray examination of the newborn allowed a plan of treatment which was reassuring to the distraught parents and which provided early, successful replacement of the involved extremity. Early acceptance of a prosthesis by the family and patient afforded a ...

In glaucoma, elevated intraocular pressure causes a progressive loss of retinal ganglion cells and results in optic neuropathy. The authors propose a potential mechanism for cell death, whereby elevated intraocular pressure causes fluid to permeate axonal membranes, creating a passive intracellular fluid flow within the axons. It is hypothesized that this intracellular flow locally depletes the adenosine triphosphate (ATP) concentration, disrupting axonal transport and leading to cell death.A mathematical ...

The system of algebraic equations given by $\sum_{j=0,\,j\neq i}^n\frac{sgn}(x_i-x_j)}{|x_i-x_j|^a}=1$, $i=1,2,\dots,n$, $x_0=0$, appears in dislocation theory in models of dislocation pile-ups. Specifically, the case $a=1$ corresponds to the simple situation where n dislocations are piled up against a locked dislocation, while the case $a=3$ corresponds to n dislocation dipoles piled up against a locked dipole. We present a general analysis of systems of this type for $a>0$ and n large. In the asymptotic ...
