We aimed to clarify the usefulness of measuring the flow mediated dilatation (FMD) in patients with type 2 diabetes mellitus without and with coronary heart disease (CHD). The FMD was measured in 480 patients with type 2 diabetes and in 240 nondiabetic subjects. The FMD was significantly lower in the subjects with CHD ( n = 145, 5.4 ± 3.2 %) than in those without CHD ( n = 95, 6.9 ± 3.5 %) among the nondiabetic subjects. 老年者 Sick Sinus Syndrome 例33例を対象とし, 平均3年2カ月間経過を観察し, 臨床所見, 治療と予後を検討して以下の成績を得た.1) 年齢では60歳台<70歳台<80歳台と高齢者に多い傾向を認めた. 2) 臨床的分類では徐脈頻脈症候群(15例) と洞停止あるいは洞房ブロック例 (14例) が多く, 洞徐脈例 (4例) は少数であった. 3) 全例の72.7%が症状を呈したが, 無症状例に80歳台高齢者, 老人性痴呆合併例を多く認めた. 4) 心電図上房室ブロック, あるいは脚ブロックの合併を21.2%に認めたが, 陣旧性心筋梗塞を示す例は12.1%にすぎず, 老人における慢性洞結節機能不全と心筋虚血との間に密接な関連は認められなかった. 5) 約半数の症例では薬物療法による治療が可能であったが, 42.4%の症例では, 短期あるいは長期心臓ペーシングを必要とした. また抗不整脈薬以外の薬によっても洞結節機能抑制を呈する例が認められた. 6) 観察期間中症状不変18.2%, 軽快57.6%で, 死亡率は24.2%であったが, うち不整脈死または関連ある心死は9.9%であった. ...

老年者非リウマチ性僧帽弁閉鎖不全症 (MR) 23例 (54歳～90歳, 平均77.8歳) に於て収縮期雑音, 僧帽弁逆流量, 心拍出量等に対するイソプロテレノール (ISP) の効果を検討し, 老年者MRの僧帽弁支持機構不全を中心に考察を加えた.ISPによる収縮後期雑音減少群では16例, 非減少群は7例であった. 収縮期雑音の強さは減少群平均 Levine 2.7°で非減少群2.4に比して弱くなく, 一方心不全は減少群でやや軽度であった. 心胸比は差がなく, 減少群では心筋梗塞7例 (43.7%), 非減少群では0%と心筋梗塞が多く見られた. 色素稀釈法による僧帽弁逆流量は減少群で48.9%, 非減少群で76.4%であった. ISPによる変化では心拍出量増大は減少群で大であった. 心エコー図と色素稀釈法より推定した僧帽弁逆流量も非減少群44.1ml, 減少群28.4mlと減少群で少なかった.以上老年者MRの中にはISPによる心筋収縮力増強により僧帽弁支持機構不全の改善により僧帽弁逆流を減少させうる症例を認めた. 一方, 心不全の軽快や心拡大の減少と共に収縮期雑音減少を示す症例も認められ, MRの発生機序として左室収縮力の障害も一因と考えられた.
