The effectiveness of stool-based colorectal cancer (CRC) screening, including fecal immunochemical tests (FITs), relies on colonoscopy completion among patients with abnormal results, but in safety net systems and federally qualified health centers, in which FIT is frequently used, colonoscopy completion within 1 year of an abnormal result rarely exceeds 50%. Clinician-identified factors in follow-up of abnormal FIT results are understudied and could lead to more effective interventions to address ...

Despite wide adoption of rapid response teams across the United States, predictors of in-hospital mortality for patients receiving rapid response team calls are poorly characterized. Identification of patients at high risk of death during hospitalization could improve triage to intensive care units and prompt timely reevaluations of goals of care. We sought to identify predictors of in-hospital mortality in patients who are subjects of rapid response team calls and to develop and validate a predictive ...

The most common cause of female infertility is polycystic ovary syndrome (PCOS), which occurs in 5% to 10% of reproductive-age women. Clomiphene citrate has been used as a fertility agent for decades and is the current first-line infertility treatment in women with PCOS. However, this selective estrogen-receptor modulator has a number of drawbacks. In addition to its overall poor efficacy, clomiphene is associated with a high multiple-pregnancy rate compared with unassisted conception and an undesirable ...

Clomiphene is the current first-line infertility treatment in women with the polycystic ovary syndrome, but aromatase inhibitors, including letrozole, might result in better pregnancy outcomes.

Visual memory has been demonstrated to play a role in both visual search and attentional prioritization in natural scenes. However, it has been studied predominantly in experimental paradigms using multiple two-dimensional images. Natural experience, however, entails prolonged immersion in a limited number of three-dimensional environments. The goal of the present experiment was to recreate circumstances comparable to natural visual experience in order to evaluate the role of scene memory in guiding ...

The US EPA Worker Protection Standard requires pesticide safety training for farmworkers. Combined with re-entry intervals, these regulations are designed to reduce pesticide exposure. Little research has been conducted on whether additional steps may reduce farmworker exposure and the potential for take-home exposure to their families. We conducted an intervention with 44 strawberry harvesters (15 control and 29 intervention group members) to determine whether education, encouragement of handwashing, ...

In safety-net healthcare systems, colonoscopy completion within 1-year of an abnormal fecal immunochemical test (FIT) result rarely exceeds 50%. Understanding how electronic health records (EHR) documented reasons for missed colonoscopy match or differ from patient-reported reasons, is critical to optimize effective interventions to address this challenge. We conducted a convergent mixed-methods study which included a retrospective analysis of EHR data and semi-structured interviews of adults 50-75 ...

Many clinical investigators think that the burden of Institutional Review Board (IRB) requirements has been consistently increasing over recent years, although there are few objective data describing these trends. Over a period of 7 years, the Reproductive Medicine Network observed a significant increase in the size and requirements of IRB submissions and significant variability of IRB performance in reviewing multicenter trials. These additional regulatory and administrative demands represent substantial ...

BACKGROUND Opioids and benzodiazepines are frequently used in hospitals, but little is known about outcomes among ward patients receiving these medications. OBJECTIVE To determine the association between opioid and benzodiazepine administration and clinical deterioration. DESIGN Observational cohort study. SETTING 500‐bed academic urban tertiary‐care hospital. PATIENTS All adults hospitalized on the wards from November 2008 to January 2016 were included. Populations of the eastern chipmunk (Tamias striatus) were studied during the years 1962 to 1970 at locations in Pennsylvania and Vermont by means of repetitive live-trapping of gridded plots. Analysis of the resultant data provided life tables, survivorship curves, and population age classes for this species. Calculations were based on disappearance of individuals from the trapped plots rather than on known mortality. Four thousand individuals were trapped of which 1580 were first captured as juveniles. ...

The design and construction of the photo-recorder described here were prompted by the need to make continuous records of small mammal and bird activity where the presence of an observer or traps would involve undesired variables and costly expenditure of time. The recorder had to be completely portable, compact, and capable of operating several consecutive days without attention. 0. P. Pearson (1959. A traffic survey of Microtus-Reithrodontomys runways. J. Mamm., 40:169-180) has described and illustrated ...

Abstract Small mammal distribution was examined in poletimber and sawtimber stands of four forest cover types in northern New England: northern hardwoods, red maple, balsam fir, and red spruce-balsam fir. During 1980 and 1981, eight stands on the White Mountain National Forest, NH, were sampled with four trap types (three sizes of snap traps and one pit-fall) for 16 000 trap-nights. Trapping was conducted during four 5-day periods in June and July. A total of 1206 individuals of seven species — Blarina ...

Mass surveillance testing can help control outbreaks of infectious diseases such as COVID-19. However, diagnostic test shortages are prevalent globally and continue to occur in the US with the onset of new COVID-19 variants and emerging diseases like monkeypox, demonstrating an unprecedented need for improving our current methods for mass surveillance testing. By targeting surveillance testing toward individuals who are most likely to be infected and, thus, increasing the testing positivity rate ( ...

Background and Objectives: Collection of feedback regarding medical student clinical experiences for formative or summative purposes remains a challenge across clinical settings. The purpose of this study was to determine whether the use of a quick response (QR) code-linked online feedback form improves the frequency and efficiency of rater feedback. Performing interstage home monitoring using digital platforms (teleIHM) is becoming commonplace but, when used alone, may still require frequent travel for in-person care. We evaluated the acceptability, feasibility, and added value of integrating teleIHM with synchronous telemedicine video visits (VVs) and asynchronous video/photo sharing (V/P) during the interstage period. We conducted a descriptive program evaluation of patient-families receiving integrated multimodality telemedicine (teleIHM + VV + ...

Pentaerythritol tetranitrate (PETN) is a very common nitrate ester explosive that has been widely studied due to its use in military and commercial explosives. Recent experimental work and calculations have shown that substituting the central carbon atom of PETN with a silicon atom results in an extremely sensitive contact explosive. We have attempted to develop PETN derivatives which are less sensitive, by attaching hydrogen, amino, and methyl groups to the central carbon atom, and substituting ...
