This article considers spoken interaction in semi-structured qualitative research interviews, comparing those that are conducted by telephone or face-to-face. It draws upon recent empirical research that illuminated some of the differences that may be observed between these two interview modes. Methodological techniques drawn from Conversation Analysis were used to conduct a systematic and transparent comparison of the interview interactions, focusing on the spoken interactional devices that researcher ...

When an object we represent in visual working memory (VWM) changes, its representation is modified accordingly. VWM can either access and change the existing representation by an updating process, or it can reset, by encoding the object in its novel status as a new representation. Our goal was to show that the determining factor of updating versus resetting is the availability of a stable correspondence between the object and its VWM representation. Here, we demonstrate that updating relies on the ...

To accomplish even rudimentary tasks, our cognitive system must update its representation of the changing environment. This process relies on visual working memory (VWM), which can actively modify its representations. We argue that this ability depends on a pointer system, such that each representation is stably and uniquely mapped to a specific stimulus. Without these pointers, VWM representations are inaccessible and therefore unusable. In three Electroencephalogram (EEG) experiments, we examined ...

Summary Background Stroke thrombolysis with alteplase is currently recommended 0–4·5 h after stroke onset. We aimed to determine whether perfusion imaging can identify patients with salvageable brain tissue with symptoms 4·5 h or more from stroke onset or with symptoms on waking who might benefit from thrombolysis. Methods In this systematic review and meta-analysis of individual patient data, we searched PubMed for randomised trials published in English between Jan 1, 2006, and March 1, 2019. We also ...

Background Patients who have had a stroke with unknown time of onset have been previously excluded from thrombolysis. We aimed to establish whether intravenous alteplase is safe and effective in such patients when salvageable tissue has been identified with imaging biomarkers. Methods We did a systematic review and meta-analysis of individual patient data for trials published before Sept 21, 2020. Randomised trials of intravenous alteplase versus standard of care or placebo in adults with stroke ...

Background Nerinetide, an eicosapeptide that interferes with post-synaptic density protein 95, is a neuroprotectant that is effective in preclinical stroke models of ischaemia-reperfusion. In this trial, we assessed the efficacy and safety of nerinetide in human ischaemia-reperfusion that occurs with rapid endovascular thrombectomy in patients who had an acute ischaemic stroke. Methods For this multicentre, double-blind, randomised, placebo-controlled study done in 48 acute care hospitals in eight ...

Importance Intravenous thrombolysis with tenecteplase improves reperfusion prior to endovascular thrombectomy for ischemic stroke compared with alteplase. Objective To determine whether 0.40 mg/kg of tenecteplase safely improves reperfusion before endovascular thrombectomy vs 0.25 mg/kg of tenecteplase in patients with large vessel occlusion ischemic stroke. Design, Setting, and Participants Randomized clinical trial at 27 hospitals in Australia and 1 in New Zealand using open-label treatment and blinded ...

Background and Purpose— Reversal of dabigatran before intravenous thrombolysis in patients with acute ischemic stroke has been well described using alteplase but experience with intravenous tenecteplase is limited. Tenecteplase seems at least noninferior to alteplase in patients with intracranial large vessel occlusion. We report on the experience of dabigatran reversal before tenecteplase thrombolysis for acute ischemic stroke. Methods— We included consecutive patients with ischemic stroke receiving ...

Individuals with minor ischaemic stroke and intracranial occlusion are at increased risk of poor outcomes. Intravenous thrombolysis with tenecteplase might improve outcomes in this population. We aimed to test the superiority of intravenous tenecteplase over non-thrombolytic standard of care in patients with minor ischaemic stroke and intracranial occlusion or focal perfusion abnormality.
