Obstructive sleep apnea (OSA) syndrome is a multi-factorial disorder. Recently identified pathophysiological contributing factors include airway collapsibility, poor pharyngeal muscle responsiveness, a low arousal threshold, and a high loop gain. Understanding the pathophysiology is of pivotal importance to select the most effective treatment option. It is well documented that conventional treatments (continuous positive airway pressure (CPAP), upper airway surgery, and dental appliance) may not always ...

Obstructive sleep apnea syndrome (OSA) is a multi-factorial disorder, with quite complex endotypes, consisting of anatomical and non-anatomical pathophysiological factors. Continuous positive airway pressure (CPAP) is recognized as the first-line standard treatment for OSA, whereas upper airway (UA) surgery is often recommended for treating OSA patients who have refused or cannot tolerate CPAP. The main results achievable by the surgery are UA expansion, and/or stabilization, and/or removal of the ...

Abstract. Modeling of Cultural Heritage in a BIM environment, and in general of existing buildings, requires special attention because there are two diametrically opposed possibilities. On the one hand the attempt is to realize a very complex and accurate model, in order to provide the most comprehensive representation of the architecture as possible. The opposite position leads to build a very schematic and symbolic model of as-built architecture. It is not easy to determine which is the right ...

Joint diseases in antiquity and the Renaissance were generally known by the all-encompassing term, gout (podagra or gotta). Only in later centuries was there a differentiation in the types of joint diseases, distinguishing gout in the modern sense from other arthritic and rheumatic disorders. The present article illustrates one pictorial representation of joint disease from the early sixteenth century, a case that seems typical of gouty tophi.

Abstract Since the time of its identification, the natural compound largazole rapidly caught the attention of the medicinal chemistry community for its impressive potency as an inhibitor of histone deacetylases (HDACs) and its strong antiproliferative activity against a broad panel of cancer cell lines. The design of largazole analogues is an expanding field of study, due to their remarkable potential as novel anticancer therapeutics. At present, a large ensemble of largazole analogues has been reported, ...

Objectives EGFR T790M mutation is the most common mechanism of resistance to first-/second-generation EGFR tyrosine kinase inhibitors (TKIs) in non-small cell lung cancer (NSCLC) and could be overcome by third-generation EGFR-TKIs, such as osimertinib. Liquid biopsy, a non-invasive technique used to test the presence of the resistant mutation, may help avoiding tissue re-biopsy. However, analysing only circulating-free DNA, information about other less frequent and coexisting resistance mechanisms ...
