Dilated cardiomyopathy (DCM) is a major cause of heart failure, characterized by ventricular dilatation and systolic dysfunction. Familial DCM is reportedly caused by mutations in more than 50 genes, requiring precise disease stratification based on genetic information. However, the underlying genetic causes of 60 to 80% of familial DCM cases remain unknown. Here, we identified that homozygous truncating mutations in the gene encoding Bcl-2–associated athanogene (BAG) co-chaperone 5 (BAG5) caused inherited ...

Hand, foot, and mouth disease (HFMD) is an acute febrile illness characterized by fever; sore throat; and vesicular eruptions on the hands, feet, and oral mucosa. Until 2010, HFMD was predominantly associated with enterovirus (EV) A71 and coxsackievirus (CV) A16 in Japan. In 2011, CV‐A6 emerged as a primary causative agent, causing the largest HFMD epidemic in Japan since 1981. Since then, CV‐A6 has caused large HFMD epidemics every 2 years. The phylogenetic analysis of complete Viral Protein 1 (VP1) ...

This paper shows that, in a pair of one-dimensional complex Ginzburg-Landau (CGL) systems, diffusive connections can induce amplitude death. Stability analysis of a spatially uniform steady state in coupled CGL systems reveals that amplitude death never occurs in a pair of identical CGL systems coupled by no-delay connection, but can occur in the case of delay connection. Moreover, amplitude death never occurs in coupled identical CGL systems with zero nominal frequency. Based on these analytical ...

Virus-associated hemophagocytic syndrome is a fulminant disorder associated with systemic viral infection and characterized pathologically by multiple-organ infiltration of hemophagocytic histiocytes into the lymphoreticular tissues. This is the first report of a previously healthy adult in whom Human herpesvirus 6 reactivation induced this syndrome with severe hemodynamic and respiratory distress.

This study aimed to evaluate the effect of a phosphodiesterase inhibitor, cilostazol, on the prevention of silent brain infarction in diabetic patients without symptoms of vascular events. A total of 89 subjects were allocated at random to the cilostazol group (n = 43) or the control group (n = 46). After the study period (3.2 ± 0.5 years), carotid intima-media thickness (IMT) (means ± SD) had increased (p < 0.01) by 0.18 ± 0.19 mm in the control group. In the cilostazol group, intima-media thickness showed almost ...

We succeeded in miniaturizing a needle-type glucose monitoring system with characteristics suitable for application in a wearable, closed-loop control system. A wearable artificial endocrine pancreas (12 × 15 × 6 cm, 400 g) consisting of a sensor, a microcomputer system that calculates insulin and glucagon infusion rates, and two roller pumps was developed. Continuous glucose monitoring by a glucose sensor inserted in the subcutaneous tissue of the forearm or abdomen of healthy and diabetic volunteers ...

Miniaturisation of the bedside artificial endocrine pancreas is necessary to provide a means of restoring physiological glycaemic excursions in diabetic patients in the long term. One of the remaining problems in producing such a sophisticated device is the difficulty in developing a sufficiently small glucose-monitoring system. A needle-type glucose sensor has been developed which is suitable for use in a closed-loop glycaemic control system. The wearable artificial endocrine pancreas, incorporating ...

The effectiveness of insulin administration by rectal suppository was examined in normal and non-insulin-dependent nonobese diabetic subjects. A 100-U insulin suppository (mean 1.8 U/kg) given to the diabetic subjects caused four times as great a fall in plasma glucose compared with the normal subjects given the same dose (mean 1.6 U/kg). The insulin response after suppository administration demonstrated a significantly positive correlation (r = 0.83, P less than 0.01) with the plasma glucose level before ...

Patients with chronic heart failure (CHF) at risk of sudden cardiac death (SCD) are often treated with implantable cardiac defibrillators (ICDs). However, current criteria for device use that is based largely on left ventricular ejection fraction (LVEF) lead to many patients receiving ICDs that never deliver therapy. It is of clinical significance to identify patients who do not require ICDs. Although cardiac I-123 meta-iodobenzylguanidine (MIBG) imaging provides prognostic information about CHF, whether ...

Six alloxan-diabetic dogs with fasting plasma glucose levels above 200 mg/100 mL were treated with rectal administration of insulin suppositories twice a day for 6--9 days. The effectiveness was compared with that of subcutaneous insulin injections. In three diabetic dogs with fasting plasma glucose levels below 300 mg/100 mL, both insulin suppository at a dose of 20 U (2.3 U/kg) and subcutaneous insulin at a dose of 0.2 U/kg showed a similar effect in reducing fasting glucose levels and daily urinary ...

Arrhythmogenic cardiomyopathy (ACM) caused by TMEM43 p.S358L is a fully penetrant heart disease that results in impaired cardiac function or fatal arrhythmia. However, the molecular mechanism of ACM caused by the TMEM43 variant has not yet been fully elucidated. In this study, we generated knock-in (KI) rats harboring a Tmem43 p.S358L mutation and established induced pluripotent stem cells (iPSCs) from patients based on the identification of TMEM43 p.S358L variant from a family with ACM. The Tmem43- ...

Liver dysfunction has a prognostic impact on the outcomes of patients with advanced heart failure (HF). The model for end-stage liver disease (MELD) score is a robust system for rating liver dysfunction, and a high score has been shown to be associated with a poor prognosis in ambulatory patients with HF. In addition, cardiac metaiodobenzylguanidine (MIBG) imaging provides prognostic information in patients with chronic HF (CHF). However, the long-term predictive value of combining the MELD score and ...

Abstract Reactions of the title compounds with 2,3-dichloro-4,5-dicyano-p-benzoquinone were reinvestigated; it was found that the corresponding products were not the ring-contracted and rearranged thieno[2,3-b]indolizines, as described previously by us, but were ordinary dehydrogenated compounds, pyrido[2,1-c]thieno[3,2-e][1,4]thiazines. In contrast with pyrido[2,1-c]thieno[3,2-e][1,4]thiazine derivatives which have no substituent or only one methyl group on the pyridine ring, their 6,8-dimethyl ...

Abstract Various 1-pyridinio(thiocarbonyl)methylides and 2-isoquinolinio(thiocarbonyl)methylides react smoothly with dimethyl acetylenedicarboxylate at room temperature or at the reflux temperature in chloroform to give new heterocycles, dimethyl 10aH-pyrido[1,2-d][1,4]thiazepine-1,2-dicarboxylate derivatives, in moderate yields. Similarly, the reactions of some 3-(1-pyridinio)thiophene-2-thiolates with the same reagent afforded the corresponding dimethyl 6aH-pyrido[1,2-d]thieno[2′,3′-b][1,4]thiazepine- ...
