Dystonia is associated with excessive corticospinal motor output. Motor cortex excitability may be reduced by low-frequency repetitive transcranial magnetic stimulation (rTMS) of premotor cortical areas. We report the effects of 1 Hz rTMS applied at 90% of resting motor threshold over the left premotor cortex in an open pilot study of three patients with severe, generalized, secondary dystonia including painful spasms in the proximal and axial musculature. A 20-min session of premotor rTMS was daily ...

Assess the frequency of paradoxical reactions encountered in daily practice under tocilizumab, using the REGATE (Registry-RoActemra) registry. The secondary objectives were to determine the type of paradoxical reaction and the consequences of these reactions. The REGATE registry is an independent prospective registry, promoted by the French Society of Rheumatology, consisting of patients treated with tocilizumab for rheumatoid arthritis. The paradoxical reaction was retained if it was a paradoxical ...

During the pre-surgical evaluation of drug-resistant epilepsy, the assessment of the extent of the epileptogenic zone and its organization is a crucial objective. Indeed, the epileptogenic zone may be organized as a simple focal lesional site or as a more complex network (often referred to as the 'epileptogenic network') extending beyond the lesion. This distinction is particularly relevant in developmental lesions such as focal cortical dysplasias or dysembryoplastic neuroepithelial tumours and may ...

Summary: Purpose: Humming is a rare automatism occurring in partial seizures that has received little attention. Its study could shed light on the neural networks underlying melodic expression. In this study, we examined the anatomoelectroclinical correlates of humming during epileptic seizures Methods: Three patients undergoing presurgical stereoelectroencephalography (SEEG) for medically intractable temporal lobe epilepsy were studied. Coherence analysis of SEEG activity was carried out to study ...

In this short paper, accelerated three-dimensional computer simulations of vascular trees development, preserving physiological and haemodynamic features, are reported. The new computation schemes deal: (i) with the geometrical optimization of each newly created bifurcation; and (ii) with the recalculation of blood pressures and radii of vessels in the whole tree. A significant decrease of the computation time is obtained by replacing the global optimization by the fast updating algorithm allowing ...

In this paper, a model-based approach to medical image analysis is presented. It is aimed at understanding the influence of the physiological (related to tissue) and physical (related to image modality) processes underlying the image content. This methodology is exemplified by modeling first, the liver and its vascular network, and second, the standard computed tomography (CT) scan acquisition. After a brief survey on vascular modeling literature, a new method, aimed at the generation of growing three- ...

The widespread use of TNFα antagonists in recent years has led to the recognition of paradoxical adverse effects, defined as the onset or exacerbation of disorders that are usually improved by TNFα antagonists. Cutaneous psoriasis is an example, of which several cases have been reported. To identify cases of psoriasis onset or exacerbation during TNFα antagonist therapy and to look for potential predictive factors. To generate temporally-controlled targeted somatic mutations selectively and efficiently in smooth muscles, we have established a transgenic SMA-Cre-ER(T2) mouse line in which the expression of the Tamoxifen-dependent Cre-ER(T2) recombinase is under the control of a large genomic DNA segment of the mouse smooth muscle alpha actin (SMA) gene, contained in a Bacterial artificial chromosome (Bac). Abstract Objective To determine whether the −308 A/G tumor necrosis factor α (TNFα) gene polymorphism can predict the outcome of etanercept therapy in 86 patients with rheumatoid arthritis (RA), as already observed in patients treated with infliximab. Methods Eighty‐six RA patients treated with etanercept were genotyped for −308 A/G TNFα gene polymorphism by polymerase chain reaction and melting curve analysis, using specific gene primers and probes. Patients were subdivided into group A (G/A genotype) ...

Ankylosing spondylitis (AS) as a progressive inflammatory disease, usually affects the articulation of the axial skeleton. The accumulation of data indicates that T lymphocytes possess a major part in the pathogenesis of AS. Herein, we assessed the effect of PBMC exosomes from AS patients on the function and expression profile of normal T cells.Blood was obtained from individuals and exosomes were isolated from PBMCs. T cells were cultured and treated with PBMC exosomes to evaluate the expression ...

There are few treatments for reactive arthritis (ReA). Since concentrations of tumor necrosis factor α (TNFα) are high in the serum and joints of patients with persistent ReA, this cytokine could be targeted in patients who do not respond to nonsteroidal antiinflammatory drugs (NSAIDs) and disease-modifying antirheumatic drugs (DMARDs). We under-took this study to investigate the safety and efficacy of TNF antagonists in patients with recent-onset and refractory ReA.All French rheumatology and internal ...

The amount of experimental data concerning physiology and anatomy of the nervous system is growing very fast, challenging our capacity to make comprehensive syntheses of the plethora of data available. Computer models of neuronal networks provide useful tools to construct such syntheses. They can be used to interpret experimental data, generate experimentally testable predictions, and formulate new hypotheses regarding the function of the neural systems. Models can also act as a bridge between different ...

Electrical stimulation of the temporal cortex in patients with epilepsy sometimes elicits experiential phenomena such as recollection of vivid memories. The neurophysiological substrate of such phenomena is poorly understood. Furthermore, the relation between the site of stimulation and the type of memory elicited has only recently started to be investigated. We investigated these issues in patient FGA who had intracerebral electrodes stereotaxically implanted in the right temporal lobe for investigation ...

This paper presents a novel approach to identify complex shapes based on the Radon transform. We propose an adaptation of Radon transform called R-transform, which is invariant to common geometrical transformations. Moreover, to improve the uniqueness of the approach, a binary shape is projected into the Radon space for different levels of the Chamfer distance transform. The accuracy and the efficiency of the proposed algorithm in the presence of a variety of transformations are evaluated within ...

In the field of epilepsy, the analysis of stereoelectroencephalographic (SEEG) signals recorded with depth electrodes provides major information on interactions between brain structures during seizures. A comprehensive methodology of comparing SEEG seizure recordings is presented. It proceeds in three steps: 1) segmentation of SEEG signals; 2) characterization and labeling of segments; and 3) comparison of observations coded as sequences of symbol vectors. The third step reports a vectorial extension ...

We recorded with intracerebral electrodes the onset of limbic seizures in patients with mesial temporal lobe epilepsy (MTLE) to identify the dynamic interactions between the hippocampus (HIP), amygdala (AMY) and entorhinal cortex (EC).Interactions were quantified by analyzing the interdependencies between stereo-electroencephalographic (SEEG) signals using a nonlinear cross-correlation method. Seizures from 12 patients were analyzed by identifying three periods of interest: (i) the rapid discharge that ...
