Endoscopic full-thickness resection (EFTR) is an emerging technique for the treatment of various conditions for which classic endoscopic resection techniques have failed or were considered to be at high risk for perforation. The full-thickness resection device (FTRD) is an over-the-scope system which allows a single-step EFTR. The aim of our study is to describe our experience in EFTR of colorectal lesions using the FTRD.Nine patients (10 colorectal lesions) were proposed for EFTR. Safety, R0 resection ...

Narrow-band imaging (NBI) allows "in vivo" classification of colorectal polyps. We evaluated the optical diagnosis by nonexpert community-based endoscopists in routine clinical practice, the impact of training, and whether the endoscopists could achieve the threshold for the "do not resect" policy. This was an observational study performed in two periods (P1 and P2). Endoscopists had no prior experience in NBI in P1 and applied the technique on a daily basis for 1 year before participation in P2. Lesions ...

Radiofrequency ablation therapy is an effective endoscopic option for the eradication of Barrett's esophagus that appears to reduce the risk of esophageal cancer. A concern associated with this technique is the development of subsquamous/buried intestinal metaplasia, whose clinical relevance and malignant potential have not yet been fully elucidated. Fewer than 20 cases of subsquamous neoplasia after the successful radiofrequency ablation of Barrett's esophagus have been reported to date. Here, ...

Background: Obesity-related comorbidities are present in young obese children, providing a platform for early adult cardiovascular disorders. Objectives: To compare and correlate markers of adiposity to metabolic disturbances, vascular and cardiac morphology in a European pediatric obese cohort. Methods: We carried out an observational and transversal analysis in a cohort consisting of 121 obese children of both sexes, between the ages of 6 and 17 years. The control group consisted of 40 children with ...

Background and aims Subclinical intestinal inflammation is common in Crohn's disease (CD). We aimed to explore its impact in the disease progression of infliximab-treated patients and the usefulness of fecal calprotectin (FC) and C-reactive protein (CRP) as surrogate minimally invasive biomarkers. Methods The registry-based, prospective, observational, multicenter DIRECT (study to investigate the correlation of fecal calprotectin with serum Drug levels and development of an antI-dRug antibodiEs among adult patients ...

Abstract Background Timely stratification of Crohn's disease (CD) is essential for patients' management. The use of noninvasive accurate biomarkers is key to monitor treatment and to pursue mucosal healing, the ultimate treatment endpoint in CD. Objective We aimed to evaluate the performance of readily available biomarkers and develop risk matrices to predict CD progression. Methods Data from 289 CD patients receiving infliximab (IFX) maintenance therapy for 2 years was collected; those patients were ...

Introduction.

Endoscopic full-thickness resection (EFTR) with the full-thickness resection device (Ovesco Endoscopy, Tuebingen, Germany) is used for the resection of subepithelial lesions and adenomas ≤20 mm, nonresectable with the following conventional endoscopic techniques: nonlifting sign or lesions in complex anatomic locations (for example, ileocecal appendix or diverticula). In our center, we performed 16 EFTRs, 10 of them with at least 1 follow-up examination 4 to 6 months after the index procedure.

A 57-year-old man who underwent total gastrectomy for gastric adenocarcinoma presented with fever and elevated inflammatory markers on postoperative day 2. Computed tomography (CT) scan and esophagogastroduodenoscopy (EGD) were unremarkable. The patient developed an evisceration and intra-abdominal infected collection, which was managed with a new surgical intervention and antibiotic treatment. He was discharged on postoperative day 33.

Abstract Background and aims Effective management of inflammatory bowel disease (IBD) relies on a comprehensive understanding of infliximab (IFX) pharmacokinetics (PK). This study’s primary goal was to develop a robust PK model, identifying key covariates influencing IFX clearance (CL), while concurrently evaluating the risk of disease progression during the maintenance phase of IBD treatment. Methods The multicenter, prospective, real-world DIRECT study was conducted in several care centers, which ...

Aim: The aim of the study was to evaluate the impact of percutaneous endoscopic gastrostomy (PEG) placement in the nutritional status of patients with oesophageal cancer requiring chemoradiotherapy (CRT). Methods: A comparative study with a prospective arm and a historical cohort was conducted. Oesophageal cancer patients undergoing CRT with dysphagia grade >2 and/or weight loss > ...

ABSTRACT Background and aims Predicting the treatment outcomes of biological therapies is an unmet need in Crohn's Disease. In this study, we explored the potential of serum neutrophil‐related biomarkers to predict infliximab therapeutic results and disease progression in Crohn's Disease patients, over a 2‐year period, in a real‐world setting. Methods The study included 100 asymptomatic Crohn's Disease patients in the IFX maintenance phase from the prospective, observational, multicenter DIRECT ...

The intravesical BCGtherapy is an efficient and recognized treatment for superficial bladder tumors and their repetitions. It is usually well tolerated (in 95% cases) but it can sometimes bring about fearful side effects. We report the case of a 74-year-old man under observation, treated for a superficial bladder tumor by BCG-therapy. Following the fourth course, the patient developped a sepsis and a multiorgan failure witch led to his death on the fifth day of his hospitalization in spite of the ...
