OVER the last 50 years, people have become increasingly concerned that the dying process is too often needlessly protracted by medical technology and is consequently marked by incapacitation, intolerable pain, and indignity. In one public opinion poll, 68% of respondents believed that "people dying of an incurable painful disease should be allowed to end their lives before the disease runs its course." 1 A number of comparable surveys indicate similar public sentiment. 2 Since the turn of the century, ...

The American Medical Association's Council on Ethical and Judicial Affairs recently reviewed the ethical implications of sexual or romantic relationships between physicians and patients. The Council has concluded that (1) sexual contact or a romantic relationship concurrent with the physician-patient relationship is unethical; (2) sexual contact or a romantic relationship with a former patient may be unethical under certain circumstances; (3) education on the ethical issues involved in sexual misconduct ...

RECENT evidence has raised concerns that women are disadvantaged because of inadequate attention to the research, diagnosis, and treatment of women's health care problems. In 1985, the US Public Health Service's Task Force on Women's Health Issues reported that the lack of research data on women limited understanding of women's health needs. 1 One concern is that medical treatments for women are based on a male model, regardless of the fact that women may react differently to treatments than men ...

This report analyzes the ethical issues raised by requirements that parents be involved when minors seek an abortion. Parents are generally supportive and understanding and can provide helpful guidance to their children. In some cases, however, parents may respond abusively to the knowledge that their minor child is pregnant or is considering an abortion. In addition, privacy in matters of health care is a profound need of minors as well as adults. Accordingly, the Council concludes that, while ...

In this report, the Council on Ethical and Judicial Affairs revisits the question of referral of patients to medical facilities in which physicians have financial interests ("self-referral"). The Council issued safeguards in 1986 to prevent abuses of self-referral and most recently updated the guidelines in 1989. Recent studies, however, have suggested that problems with self-referral persist; these problems undermine the commitment of physicians to professionalism. The Council has concluded that, ...

Summary: Forty extraarticular subtalar arthrodeses with internal fixation were performed in 26 patients with cerebral palsy who had a mobile, paralytic planovalgus deformity. We assessed the correction achieved and maintained by evaluating the clinical status, improvement in gait, correction of deformity, and foot stability. Union was achieved in a mean interval of 10 weeks with no instance of mechanical failure of the fixation screw. At a mean follow-up of 30 months, 38 feet (95%) were rated excellent ...

OVER THE NEXT 15 YEARS, under the auspices of the federal government's "human genome project," scientists will try to map in detail each of the human cell's estimated 100 000 genes. 1 The knowledge derived from the project will enable physicians to detect an increasing number of diseases and predispositions for disease. It is expected that researchers will identify genes that contribute to the development of Alzheimer's disease, alcoholism, coronary artery disease, the different forms of cancer, ...

On December 3,1990, after a year of deliberations, the Council on Ethical and Judicial Affairs of the American Medical Association issued its opinion on gifts to physicians from industry. The opinion, which follows below, has been incorporated into the American Medical Association's code of ethics for the medical profession. Many gifts that are given to physicians by companies in the pharmaceutical, device, and medical equipment industries serve an important and socially beneficial function. For ...

A type-and-screen procedure was established whereby obstetric patients with no complications would be tested for ABO, Rh, and unexpected antibody. If the antibody screen was negative, cross matches would not be performed. Type and screen could be converted in 20 minutes to cross match, or type-specific blood would be immediately available. Our initial results with type and screen demonstrated 65.5% utilization but 399 cross matches were done for 17 transfusions (cross match/transfusion ratio of 17. ...

This paper presents a generic framework of Model Driven Design (MDD) with its application for a twin screw granulation process using a mechanistic-based population balance model (PBM). The process kernels including nucleation, breakage, layering and consolidation are defined in the PBM. A recently developed breakage kernel is used with key physics incorporated in the model formulation. Prior to granulation experiments, sensitivity analysis of PBM parameters is performed to investigate the variation ...
