January 1981 through February 1985, 241 patients with rheumatic mitral valve disease (mean age 21.5 ± 11.8 years) were subjected to comprehensive mitral valvuloplasty. One hundred seven patients (44.4%) were 15 years or younger and 63 (26.1%) were 12 years or younger. One hundred seventy five patients had pure or predominant regurgitation (mean age 19.3 ± 10.7 years) and 40 (16.6%) had active rheumatic carditis at the time of the operation. Almost all patients (229) were in New York Heart Association Functional ...

The operative results of 249 mitral valvular reoperations performed in 215 patients from 1974 through 1986 were reviewed to identify groups at greater risk. Two to 4 reoperations were performed in 28 patients. The mean interval between the first and second valve procedures was 47 ± 40 months. The most common indications for reoperation were degeneration of bioprostheses (44%) and thrombosis of mechanical prostheses (26%). Thirty patients (12%) died. This rate was significantly higher (p < 0.02) than that ...

For many patients suffering from end-stage heart failure, heart transplantation remains the only hope for survival, but the shortage of donor organ is increasing. The growing number of patients awaiting heart transplantation has led many centers to expand the donor pool by liberalizing donor criteria, including advances in surgical techniques on the donor heart, such as valve repair. We subjected 4 donor hearts to bench repair of the mitral valve. The first heart was from a 35-year-old woman whose ...

The importance of preservation of the subvalvular apparatus (PSVA) during mitral valve replacement (MVR) in non-rheumatic mitral valves is well recognized. Our aim was to analyse the impact of PSVA in MVR for rheumatic valves on long-term survival. From January 1992 to December 2012, 605 consecutive patients with rheumatic mitral valve disease were submitted to MVR. PSVA (limited to the posterior leaflet) was achieved in 224 (37.7%) patients. Follow-up was 4259 patient-years, and complete for 97% of ...

This study focuses on the investigation of the kinetics of municipal solid waste composting in three full-scale mechanical-biological treatment (MBT) plants. The aims were to test a kinetic model based on volatile solids (VS) content change for describing the composting process in MBT plants, and to identify the model parameters that affected the estimation of the reaction rate constant most. To achieve this, VS content and several environmental conditions, namely temperature, moisture content, oxygen ...

An animal's survival may depend on detecting new events or objects in its environment, and it is likely that the brain has evolved specific mechanisms to detect such changes. In sensory systems, neurons often exhibit stimulus-specific adaptation (SSA) whereby they adapt to frequently occurring stimuli, but resume firing when “surprised” by rare or new ones. In the auditory system, SSA has been identified in the midbrain, thalamus, and auditory cortex (AC). It has been proposed that the SSA observed ...

The reaction between [VIVOSO4] and the tetradentate N2O2-donor Schiff base ligand, N,N-bis(o-hydroxybenzaldehyde)phenylenediamine (sal-HBPD), obtained by the condensation of salicylaldehyde and o-phenylenediamine in a molar ratio of 2 : 1 respectively, resulted in the formation of [VIVO(sal-HBPD)]. The molecular structure of [VIVO(sal-HBPD)] was determined by single crystal X-ray diffraction, and confirmed the distorted square pyramidal geometry of the complex with the N2O2 binding mode of the tetradentate ...

Second-hand tobacco smoke (SHS) is a major indoor pollutant that causes serious health problems for all exposed, especially children. Children are often exposed to SHS at home, due to parental or other households' or guests' smoking. This study describes Portuguese children's exposure to SHS at home (total and by Portuguese main regions). In 2010/2011, a questionnaire was applied to a sample of Portuguese children in the 4th grade (N = 3187, mean age 9.05 ± 0.7 years, 51.1% male). Descriptive analysis, ...

The corticothalamic (CT) pathways emanate from either Layer 5 (L5) or 6 (L6) of the neocortex and largely outnumber the ascending, thalamocortical pathways. The CT pathways provide the anatomical foundations for an intricate, bidirectional communication between thalamus and cortex. They act as dynamic circuits of information transfer with the ability to modulate or even drive the response properties of target neurons at each synaptic node of the circuit. L6 CT feedback pathways enable the cortex to ...

In this review encouraged by original data, we first provided in vivo evidence that the kidney, comparative to the liver or brain, is an organ particularly rich in cysteine. In the kidney, the total availability of cysteine was higher in cortex tissue than in the medulla and distributed in free reduced, free oxidized and protein-bound fractions (in descending order). Next, we provided a comprehensive integrated review on the evidence that supports the reliance on cysteine of the kidney beyond cysteine ...

Digital preservation aims at maintaining digital objects and data accessible over long periods of time. Data grids provide several functionalities required by digital preservation systems, especially when massive amounts of data must be preserved, as in e-Science domains. We propose the use of existing data grid solutions to build frameworks for digital preservation. In this paper we survey the main threats to digital preservation, which are used to identify a central point of failure in the metadata ...

Background: The neuropeptides oxytocin and vasopressin have been repeatedly implicated in social decision making by enhancing social salience and, generally, cooperation. The iterated and sequential version of the prisoner’s dilemma (PD) game is a social dilemma paradigm eliciting strategies of cooperation versus competition. Aims: We aimed to characterise the role of PD players’ sex, game partner type (computer vs. human) and oxytocin or vasopressin inhalation on the player’s strategy preference. ...

AimsThe Inclusive Niche Hypothesis has not been validated for plants using ecophysiological performance. The few experiments have measured growth and competition but not the physiological response of plants. A metallophyte plant that hyperaccumulates aluminium (Al), Plantago almogravensis, showed a defined spatial distribution by occurring mostly on vegetation gaps associated with metalliferous areas (geochemical islands). This case was used to determine, in situ, whether the Inclusive Niche Hypothesis ...

Submitral left ventricular aneurysm is a widely recognized but relatively unknown entity that occurs almost exclusively in African patients and is often associated with mitral regurgitation. In the past 2 1/2 years, nine patients with submitral aneurysm were operated on by a new transatrial approach that exposes the neck of the aneurysm through an incision in the floor of the left atrium (roof of the aneurysm). There were four female and five male patients and the mean age was 28 +/- 3 years (range 23 ...

Bacillus stearothermophilus, a useful model to evaluate membrane interactions of lipophilic drugs, adapts to the presence of amiodarone in the growth medium. Drug concentrations in the range of 1–2 μM depress growth and 3 μM completely suppresses growth. Adaptation to the presence of amiodarone is reflected in lipid composition changes either in the phospholipid classes or in the acyl chain moieties. Significant changes are observed at 2 μM and expressed by a decrease of phosphatidylethanolamine ( ...

The demonstration of preproenkephalin A gene expression in rat dorsal root ganglia has raised the question of the physiological role of met-enkephalin-containing primary afferent fibres. Recently, we showed that systemic infection with a recombinant Herpes simplex virus encoding preproenkephalin A (HSVLatEnk1) yielded a marked increase in the density of met-enkephalin-like material synthesising neurons in rat dorsal root ganglia. This study further investigated the synthesis, transport and release ...
