A 23-year-old man had true erythrocytosis and the nephrotic syndrome. A renal biopsy specimen showed focal sclerosing glomerulonephritis and nephrosclerosis. Both serum and urinary erythropoietin levels were increased, and plasma renin activity was in the high normal range. Hemin, at concentrations optimal for globin synthesis, produces inhibition of a specific cytoplasmic protein kinase and a ribosome-associated protein kinase isolated from rabbit reticulocytes. Both enzymes are cyclic AMP-dependent. This inhibition is noncompetitive and greater when ribosomal proteins are used as the phosphate acceptors. The inhibition of these protein kinases by hemin and its partial reversal by globin suggests that hemin regulates protein kinase activity in reticulocytes.

Phosphorylation of rat liver RNA polymerase I occurred when intact rat liver nuclei were incubated with [gamma32P]ATP and N6,O2' dibutyryl cyclic 3':5'-AMP. In addition, partially purified RNA polymerase I could be phosphorylated in vitro by an endogenous protein kinase. A protein kinase from rat liver nuclei stimulates RNA synthesis. The stimulatory effect is seen primarily with nucleolar RNA polymerase and only when rat liver DNA is the template. Phosphorylation of RNA polymerase appears to be another mechanism by which protein kinases regulate gene expression.

A case involving a 65-year-old man with Waldenstrom's macroglobulinemia (WMG) which transformed into immunoblastic sarcoma is presented. Surface IgM (kappa) was demonstrated on the plasmacytoid lymphocytes of the bone marrow at presentation as well as on the less differentiated cells of the immunoblastic sarcoma. Serum IgM levels fell at the time that immunoblastic transformation occurred. This case illustrates the fact that a fall in serum paraprotein may herald a malignant transformation of WMG.

Abstract A study was made to assess the value of cobalamin deficiency detection through quantitation of urinary methylmalonic acid (MMA). Inositol hexaphosphate (IHP) binds to deoxyhemoglabin and markedly decreases the affinity of hemoglobin for oxygen. We introduce here a method for incorporating this polyphosphate into erythrocytes, thus preparing very low affinity cells for use in respiration research. The method uses dimethyl sulfoxide (DMSO) to facilitate entry of IHP. The cells are exposed to a high concentration of DMSO which is rapidly diluted with IHP solution.
