We describe a case of primary neuroendocrine carcinoma arising from the anterior vaginal wall of a 67-year-old woman. Primary neuroendocrine carcinoma of the vagina is a rare entity with only 25 previously reported cases in the literature. In previous reports, these tumors have not been distinguished from primary neuroendocrine carcinoma of the skin (Merkel cell carcinoma). The tumor was composed of cells that showed neuroendocrine-type nuclear features with hyperchromasia, nuclear molding, occasional ...

Background Compared with the general population, the incidence of young-onset (YO) colorectal cancer (CRC) is increasing. However, a significant knowledge gap exists in the clinical characteristics, treatment patterns, and outcomes for these patients. Materials and Methods Six international tertiary cancer centers conducted a retrospective study. Patients with YO CRC (aged 18-44 years) and LO CRC (aged > 44 years) diagnosed with histologically proven colorectal adenocarcinoma from June 2003 to June 2014 ...

We present a dynamical analysis of the merging galaxy cluster system Abell 2146 using spectroscopy obtained with the Gemini Multi-Object Spectrograph on the Gemini North telescope. As revealed by the Chandra X-ray Observatory, the system is undergoing a major merger and has a gas structure indicative of a recent first core passage. The system presents two large shock fronts, making it unique amongst these rare systems. The hot gas structure indicates that the merger axis must be close to the plane ...

This study investigated the change in local skin temperature in black and white mice during irradiation at 830 nm.The photostimulation effect low-level laser therapy (LLLT) (700-900 nm) is widely accepted and used. However, the exact biological mechanisms of biostimulation are not yet established.Groups of C57BL/6J and BALB/cJ mice (n = 12 in each group) were lightly anesthetized with 50% carbon dioxide and 50% oxygen. The dorsum was shaved and a 1.0 x 0.5 cm spot was marked in the same location on each ...

Abstract Habitat interactions play key roles in regulating biodiversity and ecosystem functions. This is particularly important in aquatic ecosystems, where the flow of water facilitates exchanges of energy and matter. Oyster reefs, a highly degraded habitat globally and a key focus for restoration efforts, can reduce water movement and facilitate the deposition of particles around them, affecting nutrient cycling in surrounding sediments. The effects of these reefs on sediment infauna taxonomic ...

Indeterminate fibrohistiocytic lesions of the skin share histological and immunohistochemical features of both benign fibrous histiocytoma/dermatofibroma and dermatofibrosarcoma protuberans (DFSP). Unlike dermatofibroma, DFSP harbors recurrent genetic aberrations resulting in the fusion of COL1A1 on chromosome 17 and PDGFB on chromosome 22. Because indeterminate fibrohistiocytic lesions share some features with DFSP, they were evaluated for the possible presence of COL1A1-PDGFB chimeric transcripts. ...

Background In DISCOVER, a multinational, randomised controlled trial, emtricitabine and tenofovir alafenamide compared with emtricitabine and tenofovir disoproxil fumarate showed non-inferior efficacy for HIV prevention and improved bone mineral density and renal safety biomarkers at week 48. We report outcomes analysed after all participants had completed 96 weeks of follow-up. Methods This study is an ongoing, randomised, double-blind, multicentre, active-controlled, phase 3, non-inferiority trial ...

We aimed to systematically evaluate the feasibility of integrating HIV prevention services, including pre-exposure prophylaxis (PrEP), into a family planning setting in a high-prevalence community. We used the RE-AIM Framework (Reach, Efficacy, Adoption, Implementation, Maintenance) to evaluate the integration of HIV prevention services into a family planning clinic over 6 months. Before the integration, PrEP was not offered. We implemented a staff training program on HIV PrEP. We determined the proportion ...

Objective: The optimal screening frequency of sexually transmitted infections (STIs) for MSM and transgender women (TGW) on HIV pre-exposure prophylaxis (PrEP) is unclear, with present guidelines recommending screening every 3–6 months. We aimed to determine the number of STIs for which treatment would have been delayed without quarterly screening. Design: The US PrEP Demonstration Project was a prospective, open-label cohort study that evaluated PrEP delivery in STI clinics in San Francisco and Miami, ...

Despite the importance of understanding the prevalence, causes, and consequences of conflict and violence within families, the specific risk of violence following a family member’s release from incarceration has been hard to ascertain. Research indicates that a significant percentage of persons released from incarceration will experience involvement in family violence in their life, yet it remains unclear whether this heightened risk exists due to larger family or structural contexts or whether ...

Objective adherence metrics for tenofovir (TFV) disoproxil fumarate/emtricitabine (FTC)-based pre-exposure prophylaxis (PrEP) were critical for interpretation of efficacy in PrEP clinical trials, and there is increasing interest in using drug levels to tailor interventions for reengagement and adherence. Point-of-care immunoassays for TFV, which examine short-term adherence, are in development. However, the ability of poor short-term and long-term adherence to predict future PrEP nonretention is unknown. ...

Safe and effective use of pre-exposure prophylaxis (PrEP) depends on retention in care after initial engagement. The United States PrEP Demonstration Project offered daily oral tenofovir/emtricitabine to participants in San Francisco, Miami, and Washington, D.C. for 48 weeks from 2012 to 2014.The Demo Project participants' patterns of retention were assigned to 1 of 3 categories: early loss to follow-up (ELTF) within the first 12 weeks of the study, retention throughout the study, or intermittent retention ...

To determine if a disease state simulation assignment increases empathy, comfort, and knowledge in diabetes nutrition counseling.One-hundred forty students completing ambulatory care experiential rotations over two academic years were invited to participate in the study. Students in the intervention group completed an empathy assignment, which involved developing and following a diet plan appropriate for a patient with diabetes followed by a reflection of their experiences. Students completed a ...
