Arginine deiminase (EC 3.5.3.6) was purified to homogeneity from the cell extract of Mycoplasma arginini by molecular-sieve, anion-exchange and arginine-affinity chromatographies. The purified enzyme was composed of 2 identical sub-units with a molecular weight of 45.000 and had a pI of 4.7. α-メチルスチレン (MS) とアクリロニトリル (AN) のコポリマー鎖にN-フェニルマレイミド (PMI) またはN-P-クロロフェニルマレイミド (CPMI) を3~17mol%導入した5種のMS-AN-PMI及びMS-AN-CPMIターポリマーをそれぞれ合成した. 得られたターポリマーの熱的性質に及ぼすPMI及びCPMI組成の影響を検討した結果, PMI及びCPMI組成の増加に従ってガラス転移温度 (Tg), 軟化温度 (Tsoft), 及び熱分解開始温度 (Td) が著しく上昇することを認めた. これより耐熱性に優れる環構造のマレイミド単位をMS-ANコポリマー鎖に適量導入することによりコポリマーの熱的性質を必要に応じて改善できることが明らかとなった.

当医療圏では,在宅医主導で,PCA(patient-controlled analgesia)ポンプを使用して疼痛緩和を行うための連携システムが構築されておらず,在宅でオピオイド持続注射を実施することが困難であった.在宅緩和ケア中に内服困難となった乳がん患者に対して,病院緩和ケアチームが協力しオピオイド持続皮下注を開始した症例の経験と,医師会在宅医グループ勉強会での討論を踏まえ,システムの検討を行った.その結果,1)オピオイドは在宅医が処方,2)がん性疼痛看護認定看護師が訪問看護師に同行し,患者宅でアセスメントし,薬液を交換,3)ポンプレンタルは在宅医,消耗品は病院が負担,4)在宅医が在宅悪性腫瘍患者指導管理料,病院が共同指導管理料・訪問看護指導料をそれぞれ算定,というシステム「在宅PCAらくらくパック」を構築した.約2年間で6例の患者にこのシステムを適用した.システムを利用した在宅医,訪問看護ステーションにアンケート調査を行い,おおむね有益であるとの評価を得た.
