Recurrence of drug-resistant tuberculosis (DR-TB) after treatment occurs through relapse of the initial infection or reinfection by a new drug-resistant strain. Outbreaks of DR-TB in high burden regions present unique challenges in determining recurrence status for effective disease management and treatment. In the Republic of Moldova the burden of DR-TB is exceptionally high, with many cases presenting as recurrent. We performed a retrospective analysis of Mycobacterium tuberculosis from Moldova ...

Tuberculosis (TB) diagnosis is increasingly based on the detection of Mycobacterium tuberculosis complex (MTBC) DNA in sputum using molecular diagnostic tests as the first test for diagnosis. However, sputum can be difficult to obtain in children, patients without productive cough, and the elderly and approaches testing non-sputum samples are needed. We evaluated whether TB can be detected from the oral mucosa of patients with TB. Adults with presumptive TB were examined using culture, Xpert MTB/RIF, ...

Background The WHO End TB Strategy requires drug susceptibility testing and treatment of all people with tuberculosis, but second-line diagnostic testing with line-probe assays needs to be done in experienced laboratories with advanced infrastructure. Fewer than half of people with drug-resistant tuberculosis receive appropriate treatment. We assessed the diagnostic accuracy of the rapid Xpert MTB/XDR automated molecular assay (Cepheid, Sunnyvale, CA, USA) to overcome these limitations. Methods We ...

The incidence of multidrug-resistant tuberculosis (MDR-TB) remains critically high in countries of the former Soviet Union, where >20% of new cases and >50% of previously treated cases have resistance to rifampin and isoniazid. Transmission of resistant strains, as opposed to resistance selected through inadequate treatment of drug-susceptible tuberculosis (TB), is the main driver of incident MDR-TB in these countries. We conducted a prospective, genomic analysis of all culture-positive TB cases diagnosed ...

There is still incomplete knowledge of which Mycobacterium tuberculosis (Mtb) antigens can trigger distinct T cell responses at different stages of infection. Here, a proteome-wide screen of 20,610 Mtb-derived peptides in 21 patients mid-treatment for active tuberculosis (ATB) reveals IFNγ-specific T cell responses against 137 unique epitopes. Of these, 16% are recognized by two or more participants and predominantly derived from cell wall and cell processes antigens. There is differential recognition ...

The evolution and emergence of drug-resistant tuberculosis (TB) has been studied extensively in some contexts, but the ecological drivers of these two processes remain poorly understood. This study sought to describe the joint evolutionary and epidemiological histories of a novel multidrug-resistant Mycobacterium tuberculosis strain recently identified in the capital city of the Republic of Moldova (MDR Ural/4.2), where genomic surveillance of drug-resistant M. tuberculosis has been limited thus far. ...

Since May 2016 the World Health Organization (WHO) has recommended the treatment of multidrug-resistant tuberculosis (MDR-TB) patients with a standardised treatment regimen of 9–12 months duration if patients fulfil specific eligibility criteria [1]. This shorter-course MDR-TB treatment regimen consists of a combination of seven drugs (clofazimine, ethambutol, high-dose isoniazid, kanamycin, moxifloxacin, prothionamide and pyrazinamide) for 4–6 months, followed by four drugs (moxifloxacin, clofazimine, ...

We are grateful for the response by E. Heldal to our correspondence. Similar to other reports [1] from the World Health Organization (WHO) Euro-Region we found that a high proportion (88%) of Mycobacterium tuberculosis strains from patients with multidrug-resistant (MDR)/extensively drug-resistant (XDR) tuberculosis (TB) in the Republic of Moldova had a katG 315T mutation, which is strongly suggestive that high-dose isoniazid should not be part of a standardised treatment regimen for patients with MDR/ ...

Abstract The projected trajectory of multidrug resistant tuberculosis (MDR-TB) epidemics depends on the reproductive fitness of circulating strains of MDR M. tuberculosis (Mtb) . Previous efforts to characterize the fitness of MDR Mtb have found that Mtb strains of the Beijing sublineage (Lineage 2.2.1) may be more prone to develop resistance and retain fitness in the presence of resistance-conferring mutations than other lineages. Using Mtb genome sequences from all culture-positive cases collected ...

While the goal of universal drug susceptibility testing has been a key component of the WHO End TB Strategy, in practice, this remains inaccessible to many. Rapid molecular tests for tuberculosis (TB) and antituberculosis drug resistance could significantly improve access to testing. In this study, we evaluated the accuracy of the Akonni Biosystems XDR-TB (extensively drug-resistant TB) TruArray and lateral-flow-cell (XDR-LFC) assay (Akonni Biosystems, Inc., Frederick, MD, USA), a novel assay that detects ...

Background Limited access to drug-susceptibility tests (DSTs) and delays in receiving DST results are challenges for timely and appropriate treatment of multi-drug resistant tuberculosis (TB) in many low-resource settings. We investigated whether data collected as part of routine, national TB surveillance could be used to develop predictive models to identify additional resistance to fluoroquinolones (FLQs), a critical second-line class of anti-TB agents, at the time of diagnosis with rifampin-resistant ...

The influence of the standing wave amplitude on the resonance fluorescence of a three-level system of radiators is discussed. Great attention is devoted to the peculiarities of dressed states in the standing wave and the exchange integrals between two atoms situated in the anti-nodes or the nodes. The correlation functions of emitted photons at two dressed frequencies were obtained. It is shown that in the case when the distance between two atoms is smaller than the wavelength of the field, the ...

BACKGROUND Diagnosing drug resistance is critical for choosing effective TB treatment regimens. Next-generation sequencing (NGS) represents an alternative approach to conventional phenotypic drug susceptibility testing (pDST) for diagnosing TB drug resistance. METHODS We undertook a budget impact analysis estimating the costs of introduction and routine use of NGS in the Moldovan National TB Programme. We conducted an empirical costing study and collated price and operating characteristics for NGS ...

Background Emerging evidence suggests that shortened, simplified treatment regimens for rifampicin-resistant tuberculosis (RR-TB) can achieve comparable end-of-treatment (EOT) outcomes to longer regimens. We compared a 6-month regimen containing bedaquiline, pretomanid, linezolid, and moxifloxacin (BPaLM) to a standard of care strategy using a 9- or 18-month regimen depending on whether fluoroquinolone resistance (FQ-R) was detected on drug susceptibility testing (DST). The second-order coherent function at two-photon cooperative emission is investigated. As was observed the co-operative generation of γ-ray light is possible if the duration of X-ray pulse is longer than the co-operative delay super-radiant time of gamma photons. In this case, the amplitude of the X-ray field is constant in the process of co-operative γ-photon emission and the quasi-energetic levels for the extended nuclei system are dressed. Achievement of viable two-photon sources of correlated ...

The effect of standing wave amplitude on the resonance fluorescence of a three-level system of radiators is discussed. The cooperative emission and the fluorescence spectrum of emitted photons from two atoms situated in the nodes and anti-nodes of the laser field are investigated. A realizable model for producing an entangled state, that can be controlled via the distance of localization between two atoms in the anti-nodes, is discussed.

The resonance fluorescence emitted by two three-level atoms dressed in a strong laser field is studied. A stark dynamics shift based on photon statistics is discussed. The appearance of coherent Rabi oscillations (quantum beats) of the atoms located in the non-symmetric positions of the anti-nodes and the modification of the quantum correlations at the Stokes and anti-Stokes cooperative emission are described. For some positions of the atoms, we observed the violation of the Cauchy–Schwarz inequality, ...
