〔目的〕本研究は,CS-30を虚弱高齢者用に修正したFrail CS-10の有用性について,虚弱高齢者の下肢筋力や歩行能力との関連性から検討した。〔対象〕虚弱もしくは軽度要介護高齢者117名(男性54名,女性63名),平均年齢は77.9±9.0歳であった。〔方法〕Frail CS-10と大腿四頭筋筋力および歩行速度やTUGとの関連について,性別毎にピアソンの相関係数を求めて検討した。〔結果〕Frail CS-10は下肢筋力の指標とした大腿四頭筋筋力,歩行能力および立位バランス能力の指標とした歩行速度やTUGとの間に,男女ともに有意な相関が認められた。〔結語〕Frail CS-10は虚弱高齢者の下肢筋力のみならず,歩行能力やバランス能力をも反映する簡便なテスト法である可能性が示唆された。

本研究は，地域在住高齢者男性21名（平均年齢70.7±5.4歳）を対象に，開眼片足立ち保持時間，上下肢筋力，柔軟性などの測定と転倒歴の調査から，開眼片足立ちで30秒間保持できることの臨床的意義について検討した。片足立ち位での保持が30秒間可能であった11名と不可能であった10名を比較すると，足把持力は30秒間保持可能群が有意に強かったが，握力，足関節背屈角度，柔軟性には有意差は認められなかった。また，本研究における転倒経験者は，すべて30秒間の片足立ち保持が不可能な者であり，片足立ち保持が30秒可能な者には，最近1年間に転倒した者はいなかった。これらの結果から，地域在住高齢者男性において，開眼片足立ちが30秒間保持可能であれば，転倒を予防できる可能性が示唆された。

本研究は，地域在住の女性高齢者56名（平均年齢74.8±6.3歳）の開眼片足立ち保持時間を測定し，上下肢筋力や柔軟性，足底感覚などの身体機能評価ならびに注意機能との関連を検討した。重回帰分析によって，片足立ち保持時間に影響を及ぼす因子として抽出されたのは，足把持力と年齢であり，足把持力が強いほど，また年齢が若いほどに，片足立ち保持が安定していることが確認された。今回の知見より，地域在住女性高齢者の片足立ち能力を高めるためには，足把持力を高めることの重要性が示された。

〔目的〕本研究では,地域在住女性高齢者を対象に下肢筋力や歩行能力,平衡機能などの身体機能評価を総合的に行い,握力との関連を検討した.〔対象〕F県F町に居住する女性高齢者265名,平均年齢73.7±6.4歳であった.〔方法〕大腿四頭筋筋力,足把持力,片足立ち保持時間,長座体前屈距離,最大歩行速度,6MWT,10 m障害物歩行時間,TUG,上体起こしなどの各種身体機能と握力との関連を調査した.〔結果〕握力と有意な相関を示したのは,足把持力,大腿四頭筋筋力,片足立ち保持時間,最大歩行速度,TUG,10 m障害物歩行時間,6MWTであり,長座体前屈距離とは有意な相関は認められなかった.〔結語〕握力の測定は,地域在住女性高齢者の下肢筋力,立位バランス,応用歩行能力までを含めた全身的な体力を反映する,簡便で有用なテスト法であることが示唆された.

〔目的〕転倒に関する1年間の前向き調査を行い，地域在住高齢者の転倒要因を検討した。〔対象〕重度の認知症がなく，要介護認定を受けていない高齢者133名（平均年齢73.9±5.6歳）とした。〔方法〕身体・認知・心理機能をベースライン調査として評価した後，1年間の転倒の有無別に比較した。〔結果〕転倒を経験した35名と経験しなかった98名のベースライン時の特性比較において，足把持力，片足立ち保持時間，老研式活動能力指標，注意機能，主観的健康感の5項目に有意差が認められ，転倒経験群が非経験群より有意に劣っていた。ただし，転倒経験の有無を目的変数としたロジスティック回帰分析の結果では，すべての項目に有意なオッズ比は認められなかった。〔結語〕比較的健康度の高い地域在住高齢者では，特定の身体・認知・心理機能と転倒との関連は認められるものの，それぞれの要因が単独では転倒を引き起こす決定要因ではないことが示唆された。

〔目的〕虚弱高齢者を対象に,Timed up and go test(TUG),歩行速度,下肢機能との関連について検討した。〔対象〕虚弱高齢者134名(男性60名,女性74名)であり,平均年齢は78.4±8.6歳であった。〔方法〕TUG,歩行速度のそれぞれと下肢筋力や下肢荷重力などの下肢機能検査項目との関連を性別にピアソンの相関係数により検討した。〔結果〕TUGと歩行速度は,ともにすべての下肢機能検査項目と有意な相関が認められ,虚弱高齢者の下肢機能を把握する評価尺度としての妥当性が示された。その相関係数から関連の強さを判断すると,すべての項目で歩行速度の方がTUGよりも関連が強かった。〔結語〕TUGより歩行速度の方が,虚弱高齢者の下肢機能をより反映することが示唆された。

〔目的〕本研究の目的は，高齢者における二重課題条件下での歩行機能に，注意機能が関与しているのかを検討することである。〔対象〕対象は要支援，要介護状態にない地域在住高齢者161名である。〔方法〕5 m 歩行時間（ST歩行），計算（serial-1）を行いながらの5 m 歩行時間（DT歩行），ST歩行からDT歩行への変化量（ Δ歩行時間），注意機能（trail making test part A：TMT-A），それに認知機能（mini mental state examination：MMSE）を測定し，DT歩行はどのような因子の影響を受けているのかを検討した。〔結果〕重回帰分析の結果， Δ歩行時間はTMT-A，MMSEと有意な相関関係を示したのに対し，ST歩行時間とは有意な相関関係にはなかった。〔結語〕DT条件下での歩行能力の低下には，一般的な歩行能力ではなく注意機能や認知機能が関与していたことが示唆された。
