Samples from 450 homes with shallow private wells throughout the state of Wisconsin (USA) were collected and analyzed for 44 individual per- and polyfluoroalkyl substances (PFAS), general water quality parameters, and indicators of human waste as well as agricultural influence. At least one PFAS was detected in 71% of the study samples, and 22 of the 44 PFAS analytes were detected in one or more samples. Levels of PFOA and/or PFOS exceeded the proposed Maximum Contaminant Levels of 4 ng/L, put forward ...

Altered respiratory rate is one of the first symptoms of medical conditions that require timely intervention, e.g., sepsis or opioid-induced respiratory depression. To facilitate continuous respiratory rate monitoring on general hospital wards a contactless, non-invasive, prototype monitor was developed using frequency modulated continuous wave radar. We aimed to study whether radar can reliably measure respiratory rate in postoperative patients. In a diagnostic cross-sectional study patients were ...

Propofol, a short-acting hypnotic drug, is increasingly administered by a diverse group of specialists (e.g., cardiologists, gastroenterologists) during diagnostic and therapeutic procedures. Standard monitoring during sedation comprises continuous pulse oximetry with visual assessment of the patient's breathing pattern. Because undetected hypoventilation is a common pathway for complications, capnographic monitoring of exhaled carbon dioxide has been advocated. We examined whether the use of capnography ...

Airway mucins are the major molecular constituents of mucus. Mucus forms the first barrier to invading organisms in the airways and is an important defense mechanism of the lung. We confirm that mucin concentrations are significantly decreased in airway secretions of subjects with cystic fibrosis (CF) who have chronic Pseudomonas aeruginosa infection. In sputum from CF subjects without a history of P. aeruginosa, we found no significant difference in the mucin concentration compared to mucus from ...

A 37-y-old male was admitted to the ICU because of meningitis and respiratory failure with epileptic seizures. Spinal fluid grew Streptococcus salivarius. Prior to presentation the patient underwent surgical excision of a chronic toe ulcer, performed under spinal anaesthesia, which raised the suspicion of iatrogenic origin of the disease. The clinical situation deteriorated over the following d and the patient died from multi-organ failure. Careful hygiene measures are needed to prevent such a severe ...

A survey of agricultural chemicals in Wisconsin groundwater was conducted between October 2000 and April 2001 to obtain a current picture of agricultural chemicals in groundwater used for private drinking water. A stratified, random sampling procedure was used to select 336 sampling locations. Water from private drinking water wells randomly selected from within the 336 sampling locations was analyzed for 18 compounds including herbicides, herbicide metabolites, and nitrate. This report focuses ...

BackgroundWe have developed a prediction rule for the occurrence of perioperative red blood cell transfusion to help to reduce the number of unnecessary preoperative type and screen procedures. We evaluated the robustness of this prediction rule in patients from another hospital.MethodsThe rule was retrospectively applied to 1282 consecutive patients ('validation set') who underwent similar surgical procedures to the patients in the derivation study. The outcome was similarly defined as any allogeneic ...

In many patients, a ‘type and screen’ procedure is routinely performed before surgery. However, most patients are not transfused after all. Can we predict, which surgical patients will and will not be transfused, to reduce the number of these investigations? We studied 1482 consecutive surgical patients with intermediate risk for transfusion. Multivariate logistic regression modelling and the area under the Receiver Operating Characteristic curve (ROC area) were used to quantify how well age, gender, ...

In a prospective, randomized study, we have investigated the effects of two arbitrary pulse oximeter lower alarm limit (LAL) settings (90%:group 90, n:320 and 85%:group 85, n:327) on the incidence of hypoxaemia in the recovery room.In group 90, we calculated the theoretical effect of elimination of transient episodes of low pulse oximeter oxyhaemoglobin saturation 2 O ( p ) S by introducing a time delay between the onset of the alarm condition and triggering of the alarm. When only hypoxaemic episodes ...

The present study was designed to evaluate the myocardial protective effect of nicardipine (NIC) in patients with normal left ventricular (LV) function (control vs. NIC treatment group) and impaired LV function (control vs. NIC treatment group) during extracorporeal circulation for coronary artery surgery. NIC infusions were begun approximately 12 min before aortic cross clamping (AoX) at an infusion rate of 5 micrograms.kg-1.min-1 and maintained for 10 min. Prior to AoX an additional bolus of NIC 5 ...

The efficacy of nicardipine and nitroprusside in preventing poststernotomy hypertension was compared in two groups of 45 patients undergoing coronary artery surgery. Patients were anesthetized with fentanyl, 100 micrograms/kg, and oxygen. Group N received nicardipine at an initial rate of 3 micrograms/kg/min. Group S received sodium nitroprusside at an initial rate of 1 microgram/kg/min. The vasodilators were started before surgery, and infusion rates were adjusted to maintain systolic blood pressure ...

The effects of high doses of fentanyl (group F), sufentanil (group S), and alfentanil (group A) on posterior tibial nerve somatosensory cortical evoked potentials were studied in 30 patients scheduled for elective valve replacement surgery. Anesthesia was induced with either fentanyl, 75 microg/kg, sufentanil, 5 microg/kg, or alfentanil, 125 microg/kg. The lungs were ventilated with oxygen/air. A bolus dose of fentanyl, 25 microg/kg, was given 30 minutes after induction of anesthesia in group F. Anesthesia ...

Posterior tibial nerve somatosensory evoked potentials (PTN-SSEP) were recorded in eight patients during cardiac surgery with cardiopulmonary bypass and moderate hypothermia (25–28°C).There was no correlation between changes in amplitude and temperature; however, latencies of potentials recorded over the tibial nerve in the popliteal fossa, the lumbar spinal cord, and the cortex increased linearly as temperature decreased. Latency changes correlated well with nasopharyngeal temperature, but only poorly ...

Abstract Sodium hydroxide catalyzed glycerolysis of linseed oil is greatly enhanced by increase in temperature but only moderately by increase in catalyst concentration. That the active catalyst under these conditions is the glyceroxide ion and not the hydroxide ion was confirmed by performing the reaction with freshly prepared sodium glyceroxide. In all cases alcohol solubility is reached before the reaction attains equilibrium. Component glycerides analysis at these points shows that the monoglycerides ...

Abstract The structures of maleic anhydride adducts of essentially pure oleic, linoleic and linolenic methyl esters have been determined. The cleavage of the methyl oleate adduct yields a product with a succinyl structure, with or without shift of the double bond; four isomeric structures are possible. The first adduct of maleic anhydride with methyl linoleate is a succinyl derivative followed by a shift into the conjugated isomer with which the second maleic anhydride reacts via a 1–4 Diels Alder ...
