The history of the tranquilizer meprobamate illustrates how factors other than scientific evidence may determine physicians' patterns of drug use. Forceful advertising and publicity, an attitude of general optimism, and uncontrolled studies with favorable results combined to elevate meprobamate to the position of America's magical cure-all tranquilizer. This drug remains in wide use despite a large body of sound scientific data that questions its efficacy. Today easy pharmacologic solutions to the ...

THE neuropharmacologic properties of the many benzodiazepine derivatives available for clinical use are qualitatively similar if not identical.1 , 2 Nonetheless, new benzodiazepines continue to be marketed and promoted as "unique" agents for the treatment of anxiety or insomnia.3 Thus, there is a need for critical comparative assessment of new benzodiazepine derivatives as they are introduced to clinical use. This article assesses the properties and apparent clinical value of prazepam and lorazepam ( ...

Fourteen healthy subjects (7 male and 7 female) received 50 mg of chlordiazepoxide (CDX) hydrochloride by I‐hr intravenous infusion. Multiple venous blood samples drawn during the 72 hr after the infusion were assayed for whole blood concentrations of CDX and of its major metabolite, desmethylchlordiazepoxide (DMCDX). Mean (±SE) pharmacokinetic parameters, determined by weighted nonlinear least‐squares regression analysis, were: distribution half‐life, 1.3 (±0.3) hr; elimination half‐life, 17.8 (±2 .2) hr; ...

Ten healthy male volunteers received chlordiazepoxide (CDX) hydrochloride or matching placebo with Maalox or water in a four-way single-dose crossover trial. Coadministration of CDX with Maalox did not change the completeness of CDX absorption but significantly slowed its rate of absorption and the rate of desmethylchlordiazepoxide (DMCDX) appearance. Self-rating of feeling "spacey" at 1.0 and 2.5 hours were significantly increased over baseline for CDX taken with water but not with Maalox. Increases ...

Ten healthy male subjects ingested 25 mg of chlordiazepoxide hydrochloride (Librium) with 100 ml of water or with 100 ml of magnesium and aluminum hydroxide (Maalox) in a single‐dose crossover study. Multiple venous blood samples drawn during the first 24 hr after each dose were assayed for concentrations of chlordiazepoxide and its major metabolite, desmethylchlordiazepoxide. The antacid prolonged the mean chlordiazepoxide absorption half‐time from 11 to 24 min, and in 6 of 10 subjects delayed achievement ...

Serum creatine phosphokinase (CPK) concentrations were determined in healthy volunteers for the first 48 hours after intramuscular injection of 50 mg chlordiazepoxide hydrochloride or of its solvent alone. Intramuscular injection of both the drug solution and its solvent was painful and caused CPK elevations. The CPK rise due to the drug solution was 33 per cent higher than that due to the solvent alone, but the difference was not significant. The pH of the solvent preparation is low and it contains ...

Treatment of AnxietyNeurotic anxiety is by far the most common indication for benzodiazepines. In a three-month period, approximately one in 10 American adults will take diazepam because of tension or nervousness. Symptomatic anxiety is extremely common among the adult population, but whether it is severe or prevalent enough to justify this extent of pharmacotherapy is questionable.Anxiety is an elusive clinical syndrome. It describes a psychophysiologic response resembling fear but inappropriate ...

Physicians staffing emergency treatment facilities seldom are required to make hasty judgments regarding pharmacotherapy of drug abuse. Unnecessary morbidity frequently results from the use of barbiturates and major tranquilizers in the emergency room. The need for sedatives and tranquilizers should be carefully assessed before they are administered. Drug-induced agitation or panic will often subside spontaneously or in response to reassurance, obviating the need for coadministration of sedatives.

DERIVATIVES of the 1,4-benzodiazepine nucleus (Fig. 1) are among the most widely used drugs in clinical medicine. In 1972 more than 77 million prescriptions for benzodiazepine antianxiety agents were filled in American retail pharmacies, of which two thirds were for diazepam. Another four million prescriptions were filled for flurazepam, a hypnotic agent. The total expenditure exceeded 200 million dollars. Benzodiazepines also are extensively administered to hospitalized patients. In two Boston teaching ...

• During the removal of a ruptured arteriovenous malformation, a young man underwent partial sectioning of the splenium of the corpus callosum. Postoperative testing showed no deficit of interhemispheric visual or auditory transfer. To understand the extent of the splenial section, we cut ten normal brains in the sagittal plane and analyzed the shape of the splenium. It may be markedly curved or bulbous. Our findings lead us to conclude that a small band of ventral splenial fibers may be adequate to allow ...
