目的：人口減少と高齢化の著しい土佐清水市において，急性期医療の有効かつ円滑な運営と存続のためには，在宅医療を定着させる必要がある．方法：平成24年度在宅医療連携拠点事業を通じ，市内全医療，介護関係機関に呼びかけ（1）24時間在宅医療体制の構築（2）情報共有・連携体制の構築（3）人材育成（4）普及啓発活動に取り組んだ．結果：（1）病床（ショートステイを含む）を有する市内医療介護機関の空床情報共有を開始（2）iPadによる医療介護の情報共有システムを開発，包括支援センターとの定例カンファレンスを開始（3）多職種による合同介護研修会を行い，介護のeラーニングを作成，Web上での無料配信を開始（4）一般住民向け在宅医療啓発プログラムを作成公開，高齢者に必要な情報を一元化した資源MAPを作成配布，在宅医療推進のための公開講座を開催した．結論：一堂に会すことで，行政，医療，介護，福祉の「顔の見える関係」の構築ができ，連携が深まった．在宅医療推進活動は，地域一丸となっての「地域づくり」である．
