イブプロフェンを含有する市販薬を服用後，スティーブンス・ジョンソン症候群（Stevens-Johnson syndrome, SJS）を発症した34歳男性。副腎皮質ステロイドの大量全身投与は奏功せず中毒性表皮壊死症（toxic epidermal necrolysis, TEN）に進展し血漿交換を5回実施した。初回は新鮮凍結血漿で置換した。2～5回目はSJS/TEN発症に関わると考えられる液性因子を効率的に除去する一方で，フィブリノゲンなどの大分子量物質の除去能が低いEvacure® EC-4A（川澄化学工業）を用いアルブミンで置換する選択的血漿交換を行った。血漿交換後，皮膚症状は速やかに改善した。アルブミン製剤で置換する選択的血漿交換はSJS/TENの治療に効果を期待でき，新鮮凍結血漿で置換する血漿交換と比べ安全性および費用の点で優れている。

症例は59歳の男性. 3年前にS状結腸癌でS状結腸切除+D3郭清を受けた.平成11年3月24日より肛門痛が出現.下部消化管内視鏡でS状結腸切除の吻合部から肛門管にかけ粘膜は発赤し,白苔を有する潰瘍が多発していた.しかし吻合部より口側の粘膜には異常を認めなかった.骨盤部のCTでは直腸壁の肥厚と周囲のdirty fat signが認められた.内視鏡下の生検の組織学的検査で,好中球の浸潤と出血,浮腫が認められた.虚血性直腸炎と診断し中心静脈栄養管理したが軽快せず,横行結腸人工肛門を造設した.症状は術直後より軽快し,内視鏡所見でも正常所見になったため,人工肛門造設後6カ月で人工肛門閉鎖を行った.術後8カ月が経過しているが再発の兆候はない.

今回,われわれは腹膜垂によりイレウスを呈した症例を経験したので報告する.症例は59歳,女性.平成12年5月20日より腹痛が出現し, 5月24日,イレウスの診断で当科入院.直ちにイレウス管を挿入.小腸造影でTreitz靱帯より50cmの空腸に狭窄を認めたため,挿入6日目に開腹手術を施行.術中所見では,横行結腸の腹膜垂が長く伸び,小腸間膜に一度癒着したのち小腸壁に腹膜垂の先端が癒着して2つのループを形成,その中に小腸がそれぞれ入り込み,閉塞していた.腹膜垂を切除し,イレウスを解除したところ壊死を認めなかったため,小腸切除は行わなかった.病理所見は出血,線維化を伴う腹膜垂であった.腹膜垂によるイレウスでは,索状物による小腸の圧迫,癒着により屈曲された付着部結腸の閉塞,炎症による結腸の直接圧迫,特殊な形態により閉塞をきたす.腹膜垂によるイレウスの本邦報告例は4例であった.
