症例は69歳男性で,意識障害を主訴に来院.軽度肝機能低下,血中アンモニア高値,ICG 15分値停滞,CT上肝に軽度萎縮を認めたことから肝性脳症が疑われた.左腎静脈造影により,splenorenal-gastrorenal shuntが認められた.肝組織像ではhemosiderin沈着が見られたが,明らかな門脈圧亢進を引き起こす肝硬変や肝線維症はなかった.バルーンカテーテルによる短絡閉鎖試験を行い,閉鎖前後のアンモニア値,門脈圧の変化を測定した.短絡閉鎖後,アンモニア負荷試験においてアンモニア値は上昇せず,門脈圧は軽度上昇したことから,この短絡から門脈血が大循環系に流入していることが明らかになった.本症例のように明らかな門脈圧亢進を引き起こす肝硬変や肝線維症を伴わない門脈大循環性脳症は稀であり,短絡発生の機序として,腹部手術の癒着が成因と考えられた.

The EMPA-KIDNEY trial showed that empagliflozin reduced the risk of the primary composite outcome of kidney disease progression or cardiovascular death in patients with chronic kidney disease mainly through slowing progression.
