Resumo

Attempts at improving our management of rectal cancer engender controversies. Advances are being made in parallel with the themes seen in much of modern solid tumor management. These are the optimization of patient outcomes by (1) a detailed understanding of the pathology of rectal cancer and its relation to surgical techniques; (2) multidisciplinary management; (3) organ and function conservation; and (4) analysis of outcomes based on quality of life as well as morbidity, recurrence, and survival; and (5) treatment selection based on biologic factors. These principles are being applied in the management of patients with all stages of rectal cancer. Selection of the best route to accomplish these goals is guided by the extent of disease at presentation. Advances in management involve both clinical and translational research issues.
