Family and Patient Care in Intensive Care Units

Mechanical Ventilation at Home: (Case Report) In this report, we presented a 42-year-old man for whom mechanical ventilation at home was applied. Clinical examination at first revealed that he was confused and had respiratory failure. Diagnosis of the patient was diaphragmatic failure and stayed ta the Intensive Care Unit (ICU) 114 days. Home mechanical ventilator was provided for the patient who could not be weaned completely from mechanical ventilation during his stay in the ICU. A suitable room, electrical equipment, oxygen supply, a portable aspirator, other equipments and the family members' training were required for mechanical ventilation at home. The patient was discharged from ICU with home ventilator after the necessary equipment had been supplied and the family members trained. In the beginning he was controlled weekly, and then monthly by the physician at home. In the thirtieth month control, the patient has been undergoing mechanical ventilation only in the nights. In conclusion; mechanical ventilation at home has some advantages such as; to rise the quality of life, to decrease the length of stay in the ICU and to reduce the cost of health care. Bu raporda evde mekanik ventilasyon uygulanan 42 yasinda bir erkek hastayi sunduk. Olgunun ilk muayenesinde bilinc bulanikligi ve solunum yetersizligi vardi. Diyafragma yetersizligi tanisiyla yogun bakimda 114 gun kaldi. Yogun bakimda kaldigi sorede mekanik ven-tuatorden tamamiyla ayrilamayan olguya ev tipi mekanik ventilator saglandi. Evde mekanik ventilasyon icin uygun bir oda, elektrik donanimi, oksijen destegi, bir tasinabilir aspirator, diger donanimlar ve aile bireylerinin egitimi gerekliydi. Gerekli donanimlar saglandiktan ve aile bireyleri egitildikten sonra olgu ev tipi mekanik ventilatorle birlikte taburcu edildi. Olgu, doktor tarafindan evde baslangicta haftalik,sonra aylik olarak kontrol edildi. Otuzuncu aydaki kontrolde, olgu yalniz geceleri ventilasyon destegi aliyor.Sonuc olarak, evde mekanik ventilasyon destegi uygulamasinin; yasam kalitesini artirma, yogun bakimda kalis suresini kisaltma ve saglik bakimi maliyetlerini dusurme gibi avantajlari vardir.
