If heart failure supervenes it is usually found to be due either to involvement of the aortic valve or to narrowing of the coronary ostia. In both these instances the strain is predominantly on the left ventricle and is recognized clinically by left-sided enlargement with aortic configuration and electrocardiographic changes denoting left ventricular preponderance.A much rarer complication of aortic aneurysm is compression of the pulmonary artery with or without arterio-arterial communication.
