ResumoIsolated fracture of the greater multangular (trapezium) is rare. Due to its anatomical position and overlying carpal bones, this fracture becomes a difficult diagnostic problem. Vertical fractures of the greater multangular along the coronal axis are usually missed on routine AP, lateral, and oblique views of the wrist. Persistent disabling symptoms, abnormal physical examination with a negative standard radiographic study of the wrist should cause suspicion. Occasional attention has been drawn in the orthopaedic literature to the value of the carpal tunnel view in the detection of these fractures (Cordrey & Ferrer-Torells, 1960; McClain & Boyes, 1966; Rockwood & Green, 1975). The radiological literature does not stress this important practical application of the carpal tunnel view. We wish to present a case in which this was the only projection demonstrating this fracture.
