Thyroid Cancer Diagnosis and Treatment

Presented is the pathology of thyroid carcinomas and adenomas in 300 consecutive autopsies on patients aged 20 years and older. Eight thyroids contained primary carcinomas (incidence of 2.7%): Two which metastasized and killed the patients, were diagnosed clinically; 3 first were observed grossly at autopsy and 3 were found only on microscopic examination. Nine thyroids contained adenomas (incidence of 3.0%). The diagnostic criteria for adenomas are reviewed and the low incidence of adenomas relative to carcinomas is compared to previous studies. The malignant potential of thyroid adenoma is considered and 2 cases of "killing" cancer of the thyroid are summarized. The authors conclude that, while some carcinomas appear to arise de novo, others arise from adenomas. Because of the low incidence of adenomas relative to carcinomas and the apparent rise of some carcinomas in adenomas, the authors suggest that all true adenomas be considered as potentially malignant and be subjected to very careful gross and detailed microscopic examination.
