ResumoSince 1946, when Smol ik et al. 2 first r e p o r t e d a b ra in abscess assoc ia ted wi th congeni ta l h e a r t disease cured b y surgical d ra inage and su l fonamides , less t h a n 10 cases of comple te r ecovery f rom the so-cal led pa radox ic b ra in abscess in congeni ta l h e a r t disease have been r epor t ed . Ley et al. 1 r e p o r t e d the i r own r cases and rev iewed 6 cases which were found in the neurosurg ica l l i t e r a tu re of the l as t 10 years . Recen t ly , we have o p e r a t e d successful ly upon a p a t i e n t wi th b ra in abscess assoc ia ted wi th congeni ta l h e a r t disease of the t y p e of the t e t r a l o g y of Fa l lo t . Ca rd i a c ca the t e r i za t i on and o the r thorac icosurg ica l examina t ions were pe r fo rmed pos tope ra t i ve ly and the d e v e l o p m e n t a l mechan i sm of the pa radox ic b ra in abscess was clarified.
