ResumoPrevious studies of Medicare's diagnosis-related group (DRG)-based prospective payment system have described the effectiveness of DRGs in controlling hospital expenditures, the relationship between DRGs and nursing resources, and the fiscal implications for hospitals. Little is known about how DRGs change the health care system, particularly the nature of the changes and how they influence nursing practice and patient care. This article reports the additional findings of a study of how eight hospitals in a Midwest health systems area adapted to DRGs. Qualitative methods were used, and interviews were conducted with 118 persons affiliated with community hospitals. The findings suggest that DRGs have a critical influence on discharge planning, professional practice, and patient care.
