ResumoOver the last decade, a major culture shift has begun in surgical care with an increased willingness to acknowledge human fallibility. Surgeons have traditionally targeted the individual provider when things went wrong rather than acknowledge that we are all vulnerable to error. Solutions to prevent surgical error and patient injury were therefore based on identifying and removing the "bad apples"—those subpar providers who harmed patients—or on the idea that one just needed to "try harder" when something bad happens. In error theory, this is considered focusing on the sharp edge.
