many years has permitted behavior that once might have been seen to be rewarded in only a few. Once rewarded, it became persuasive. Once persuasive, then pervasive. A case in point is the fact, pointed out in Joseph Califano's book, America's Health Care Revolution, that the Michigan dermatologists were taking a greater share of the Michigan Blue Shield dollar than any other specialty.' I do not believe that a specialty comprising only 1% of physicians, mostly practicing honest and costeffective dermatology, could possibly do that. I would challenge Dr. Sams and others to look into the matter more closely in a scientific way, rather than to continue to make assumptions. My point is that no one has yet dared to look into our behavior and to report on it. In my opinion we are a ticking time bomb of repressed truths waiting to be exposed. Finally, I believe that academia should be more keenly aware of what I would like to call institutional as it pertains to our specialty. If the matter of ethics is not guided and protected by those in positions of power and prestige, then by whom is it guided and protected? It is from this group that studies and writing should and must come. To expect practicing dermatologists to initiate meaningful studies and change is unrealistic. Nevertheless, I appreciate Dr. Sams' comments. I am deeply concerned about our specialty and believe that all discussions on these subjects are important.
