Several studies in secondary care have suggested that H. pylori testing of dyspeptic patients referred for endoscopy (OGD) results in fewer patients needing OGD possibly because of the reassurance provided by a negative result.1.2 The Nottingham Dyspepsia Management study i s a randomized trial in primary care comparing 4 strategies: (1) immediate OGD (n=151);(2) empiric lansoprazole for 4/52(n=140); (3) H. pylori testing (n=152) and referral of H. pylori positives (Hp+ve) for OGD and (4) H. pylori testing (n=154) and treatment of Hp+ve patients with eradication therapy (n=39); patients testing H. pylori negative (Hp-ve) in the 2 testing groups were treated with lansoprazole for 4/52 (n=l10+l15).FlexSure HP ® (SKD) was used for "Near Patient" H. pylori testing.All patients were reviewed by their GP at 6 weeks with further management, including referral for endoscopy (OGD) being at the GP's discretion.Patients were followed by review of GP and hospital notes.We compared the subsequent rate of referral for OGD from the 2 groups randomized to H. pylori testing and found to be Hp-ve with the group randomized to empiric PPI treatment only.Time to OGD was compared using Kaplan Meier survival curves.Results: GroupsNo.
