Le Fort Maxillary Swing Procedure for Posterior Maxilla Tumor Extirpation

There are multiple surgical approaches that have been reported for maxillary tumor extirpation. The majority of the approaches involve transfacial incisions such as the Weber-Fergusson and lateral rhinotomy that provide exposure to the anterior and posterior portions of the maxilla, providing excellent visualization and access for control of pterygomaxillary and nasoethmoidal hemorrhage. Although often appropriate for the treatment of malignant pathology of the maxilla, extraoral approaches should be avoided when possible for the treatment of benign disease, providing that oncologic safety and vascular control can be maintained using an intraoral approach. This technical note describes the use of a Le Fort I osteotomy for identification of the tumor margins followed by maxillectomy and lateral swing of the unaffected maxilla to allow for complete posterior tumor extirpation. This modification of the traditional Le Fort I osteotomy has been described as the Le Fort I maxillary swing. A case of a large odontogenic myxoma of the maxilla is resected using this approach.
