ResumoPeriosteal and endosteal remodeling processes in the second metacarpal bone have been studied in five pathologic conditions. In disuse osteoporosis, bone apposition at the periosteal surface, concomitant with increased endosteal resorption and resulting in a lower bone mass than normal, was observed. In acromegaly, an increased bone mass was found due to increased bone apposition at the periosteal surface and to decreased resorption at the endosteal surface. The bone remodeling in hyperparathyroidism paralleled, at higher levels, the age-associated remodeling process, apposition at the periosteal surface, and resorption at the endosteal surface. In Turner's syndrome, resorption was greatly increased at the endosteal surface and, in a case of idiopathic osteoporosis, resorption at both surfaces resulted in excessive reduction of cortical bone mass.
