Resumo

Abstract Hypercarbia is discussed in terms of various types of respiratory insufficiency. Factors are outlined which predispose to, or precipitate, postoperative respiratory acidosis together with a brief discussion of its prevention and management. Selected topics of recent interest are annotated and discussed in more detail. These include the increasing importance of the bronchitic emphysema syndrome and obesity as predisposing factors, certain details of management of anesthesia and mechanical factors in the development of 2 insensitivity of the respiratory center. Difficulties of estimating alveolar ventilation are discussed. Technics for determination of P CO 2 are reviewed and recent methods utilizing alveolar gas are described. Concerning recognition and prevention the importance of preoperative lung function studies, respiratory physiotherapy and chemotherapy is emphasized. In relation to treatment the indications for, and results of, tracheostomy are described. Difficulties of artificial ventilation in obstructive disease are outlined and several new respirators are mentioned. The possible use of respiratory stimulants and of artificial respiratory paralysis is mentioned.
