ResumoPrimary biliary cystadenocarcinomas are rare malignant neoplasms of the liver. We report a case that was operated with a preoperative diagnosis of hydatid cyst of the liver. The patient underwent a wide surgical excision of the cyst. Histopathological examination revealed a hepatobiliary cystadenocarcinoma. The patient remains asymptomatic at 2 years following surgery. The importance of differentiating these rare tumors from hepatic hydatid cysts, especially in areas where the disease is endemic, is highlighted in this case report.
