To determine family physicians’ information on advance directives (AD), as well as their attitudes to and experience of these documents. A descriptive study was performed. Data were collected between April and June 2005 through a questionnaire sent to 351 family physicians in public primary care centers in Majorca (800,000 inhabitants approximately). A total of 169 family physicians completed the survey (48.1%); 97% considered that AD were useful, but 83.2% stated they had insufficient information to help their patients formalize AD. Almost all physicians (95.2%) would agree to discuss the topic when asked by a patient, 72.1% in cases of chronic disease, and 57.2% as part of routine practice. The main difficulties detected were possible legal problems (84.9%) and discrepancies between the patient's instructions and the family's opinion (80.1%). Physicians have a highly favorable attitude toward AD, but not enough practical information. Agreement on the role of healthcare professionals in the implementation of AD is lacking.
