More than 60% of such patients develop P. carinii pneumonia and about 20% have a recurrence (1). We report a case of a patient with AIDS and P. carinii pneumonia in whom a repeat biopsy after clinical response to therapy with trimethoprim-sulfamethoxazole failed to show P. carinii cysts while electron microscopy showed many trophozoites of P. carinii.A 37-year-old intravenous drug abuser was hospitalized with a 2-month history of fever and cough.
