Penetrating Pencil Injury: An Unusual Case of Child Abuse

Objective: We report an unusual case of penetrating pencil injury in a young child that turned out to be a case of physical abuse. Method: This is a case report with review of the literature. Results: A 4-year-old boy was brought by his mother to the accident and emergency department for a penetrating wound of the right hand. He was said to have injured himself during a fall at home while holding a sharpened lead pencil. The pencil broke after penetrating the hypothenar eminence, leaving the graphite tip embedded in the palm. On surgical exploration, a piece of graphite measuring 1.3 cm was removed. The injury was actually inflicted by his mother because he failed to complete his homework properly. Deep penetrating injuries associated with the lead pencil are uncommon events in the medical literature. They are often reported as accidental and usually involve the oropharynx and the orbit. Conclusion: Deep penetrating injury with a pencil is unusual in childhood and the public should be made aware of the possibility of nonaccidental injury. Résumé—Un garçon âgé de 4 ans est amené par sa mère aux urgences pour une bles par pénétration à la main droite. On qu’il s’est blessé en tombant à la maison alors qu’il tenait un crayon bien taillé. Le crayon s’est cassé après être passé à travers l’éminence hypothenare, laissant la pointe en graphite du crayon enfoncée dans la paume de l’enfant. Un morceau de graphite de cm a été extrait au cours de l’exploration chirurgicale. En fait, la blessure avait été infligée par la mère parce qu’il ne voulait pas faire ses devoirs comme il faut. Une blessure d’une telle profondeur provoquée par u crayon est inhabituelle au cours de l’enfance et la possibilité d’une blessure non-accidentelle doit être retenue. Resumen—Un niño de cuatro años fue conducido por su madre al departamento de accidentes y urgencias por una herida penetrante en la mano derecha. El ni dijo que se habı́a herido él mismo como consecuencia de una caı́da en casa mientras tenı́a en la mano un lápiz con la parte delantera afilada. El lápiz se rompio despues de penetrar en la eminencia hipotenar, dejando un trozo grafito dentro de la palma de la mano. En la exploración quirúrgica, se extrajo un trozo de grafito que medı́a 1,3 centı́metros. La herida fue realizada realmente por su madre porque el niño no habı́a completado adecuadamente los deberes del colegio. La herida penetrante profunda con lápiz no es habitual en la infancia. La posibilidad de un daño no accidental debe ser tenida en cuenta.
