ResumoSir-I have followed this correspondence with interest because we perform several thousands of these tests each year in this laboratory. I must disagree with Mr Craft and Mr Diggorys suggestion (March 24 p. 663) that semen counts are unnecessary. We have experience of 5 pregnancies following vasectomy in the Bristol clinical area and in 3 cases the post vasectomy sperm counts were carried out in this laboratory. In 2 of these 3 cases no sperm was seen in the counting chamber but careful cytological examination of stained smears showed occasional sperms in each case less than 1 in every 30 high power fields. In the 3rd case a count was reported as negative at another hospital and the follow up test in our own laboratory showed 5-6 sperm in each high power field. As a result of these experiences we prefer to examine stained smears than to rely on the counting chamber. We also prefer to examine smears from uncentrifuged rather than centrifuged specimens since the films are clearer and the sperms more easy to identify. (FULL TEXT)
