Toxic reaction following the combined administration of fluoxetine and l-tryptophan: Five case reports

Fluoxetine is an antidepressant, the chemical structure of which differs from that of other tricyclic antidepressants. It is a specific inhibitor of serotonin reuptake into neurons, with minimal effect on noradrenergic neurons (Wang et al. 197% and has been found to be efficacious in the treatment of obsessive-compulsive disorder (OCD) (Fontaine and Chouinard 1985). As serotonin has been implicated in OCD (Yaryura-Tobias and Bhagavan 1977; Fontaine et al. 1985), we recently added L-tryptophan as an adjunct to fluoxetine in 5 patients being treated for this disorder in an attempt to augment the response to fluoxetine. L-T~ptoph~ has been shown to increase central nervous system (CNS) levels of serotonin (Murphy et al. 1974) and may be effective in affective disorders (Chouinard et al. 1983) and obsessive-compulsive disorders (Yaryura-Tobias and Bhagavan 1977). However, we report the occurrence of a toxic reaction in all five cases following combined administration of fluoxetine and L-tryptophan.
