ResumoMany people suspect that aspirin intolerance is not an TgE-mediated hypersensitivity but represents a specific bioehemieal defect that, were it elucidated, might provide clues to the understanding of idiopathic asthma.Consequently, three recent reports of familial incidence of aspirin intolerence have been of considerable interest.1-aIn today's presentation, we have for discussion an additional family with asthma induced by aspirin.The presentation of the case will be by Dr. Kurt von Maur.DR. KURT VON MAUR: The proposita (IV-I in Fig. 1) is a 4%year-old black woman who was first seen in The *Johns Hopkins Hospital Allergy Clinic in August, 1972, because of episodic shortness of breath associated with wheezing over the preceding twenty years.Her asthmatic symptoms were perennial and were not predictably exacerbated during any one season of the year.They were, however, exacerbated by strong odors, upper respiratory infections, and dust exposure.Her asthmatic symptoms had never been severe enough for her to bc hospitalized.She had never smoked.She had ncvcr experienced seasonal rhinitis, urticaria, eczema, or any adverse drug reactions.Five years prior to her visit she noticed that upon ingesting aspirin or aspirincontaining compounds, she had a typical asthmatic attack beginning 15 to 30 minutes after ingestion and terminating spontaneously in 6 to 24 hours.When she recognized that aspirin was a predictable factor in her asthma, she inquired of other members of her family and discovered that several of them had the same intolerance to aspirin-containing compounds.Acetaminophen had been taken on several occasions by various aspirin-intolerant members of the family without adverse effect.lCone of them had knowingly taken tartrazine, mefenamie acid, or aminopyrine.The proposita recalled taking aspirin with impunity earlier in her life.Physical examination revealed a moderately obese woman with minimal turbinate edema and no polyps in the nose.Examination of the chest re-
