Since Kelly and Wendel first described the use of vitrectomy to repair full-thickness macular holes (MHs),1 evolving surgical experience and technique have improved anatomic success rates to greater than 90%.2–18 During the remarkable 10-year transition from having no effective treatment to having one that nearly always works, questions and controversies have arisen with the advent of numerous procedural modifications. The use of intraoperative adjuvants (transforming growth factor-beta2,8,16 serum,10 thrombin,11 platelets,12 etc.), tamponading agents (gases, air, silicone oil), and prone positioning (duration) has spurred numerous debates that remain unresolved. Regardless, surgical success has consistently increased because of, or in spite of, each "refinement."
