L'émergence et l'extension de Proteus mirabilis et de Klebsiella pneumoniae producteurs de ßlactamase à spectra élargi (BLSE), responsables essentiellement d'infections urinaires, mais également de suppurations et de septicémies, nous ont amené à proposer deux types de mesures : une attitude thérapeutique en fonction de l'état clinique du malade et de l'origine du prélèvement, traitement par le cefotetan des infections urinaires (1g × 2) en IV ou IM suivant les possibilités de la voie d'abord pendant 7 jours ou par l'imipénème (1g × 3) pendant 10 jours chez les patients à risques ou immunodéprimés. Nous avons également proposé au moment de l'isolement de vérifier le portage digestif et son éradication en fin de traitement. L'imipénème et le cefotetan ont amené une guérison clinique, le cefotetan ayant de plus assuré une décontamination digestive. Emergence and dissemination of extended-spectrum ßlactamase (ESBL) Proteus mirabilis and Klebsiella pneumoniae responsible for urinary tract infections, and also suppurations and septicemia led us to propose a therapeutic approach that was function of the clinical state and origine of the responsible strain. We proposed cefotetan treatment of urinary tract infections (1g × 2) for 7 days, IM or IV depending of the possible route, imipenem treatment (1g × 3) for 10 days in cases of septicemia or in patients who were at high risk, or immunodepressed. We proposed also once the germ was isolated from the pathological product, to test for its presence in the digestive tract and control the efficacy of digestive eradication. Imipenem and cefotetan equally provided the clinical cure, cefotetan provided also digestive decontamination.
