ResumoTwo patients had spontaneous dehiscence of an aortic commissure producing severe aortic regurgitation. There was dilatation of the aortic root in both cases which could not be attributed to inflammation, syphilis, or a connective tissue disease. Valve ring dilatation probably renders the valve susceptible to rupture by altering the relationship of the cusps to each other thus weakening their mutual support during diastole.
