Autopsy of a premature infant who died with severe respiratory insufficiency revealed a large effusion and an extralobar pulmonary sequestration in the left pleural space. The presence of dilated lymphatics in the sequestration suggested that it was the source of the effusion. The left lung was very hypoplastic, apparently at least in part as a result of the effusion.
