The US healthcare system is characterised by uneven quality, very high cost relative to health outcome, and patchy access, particularly for those lacking adequate health insurance coverage. These performance gaps in the system were described as a “chasm” in a recent report from the Institute of Medicine's committee on quality of health care.1 Many observers (including doctors and managers) agree that little research has been done on the impact of management on improved performance of healthcare delivery systems. I propose here a seven point plan to seek common ground between doctors and managers.
