Resumo

An operative procedure is described for correction of anomalous drainage of the right pulmonary veins into the superior vena cava. The procedure is technically simple and has the advantage of providing ample drainage of the pulmonary veins and superior vena cava without resorting to the use of prosthetic materials.Four case reports are presented.nomalous pulmonary venous connection is an entity reported with increased frequency since open-heart surgery and cardiac catheter-A ization have become widely used.Edwards [3] analyzed 135 patients reported up to 1953, including 5 of his own, and pointed out the anatomical and developmental features of the malformation. The earliest reports in the literature of successful surgical correction are those of Muller [I21 and Kirklin and associates [7, 81.The most common of the anomalous pulmonary venous connections is that of the right pulmonary veins draining into the right atrium or the superior vena cava (SVC) [5, 61.The anomaly may involve the right superior, right inferior, or both right pulmonary veins draining to the right atrium, and in a significant number of cases there is a concomitant atrial septal defect that may be at any level but usually is of the sinus venosus type.As pointed out by Lawrence and co-workers [9], it is quite difficult to differentiate a simple atrial septal defect from anomalous right pulmonary venous drainage on Clinical grounds, and the diagnosis is usually made at the time of cardiac catheterization. In dealing with this malformation, several surgical approaches have been proposed over the years, including ligation of the anomalous vein, resection of the related pulmonary lobe or segment, atrioseptopexy, reimplantation of the anomalous vein into the left
