The revision has benefited from comments made from the chair by Professor H. Post, from the floor by Dr D. A. Gillies, Dr J. Dorling and Dr J. Mayberry, in reply by Dr. M. L. G. Redhead and in private by Dr R. G. Wilson. The present draft has been further revised in the light of comments and suggestions made by the Editor and his three referees.
